Chest wall radiotherapy with volumetric modulated arcs and the potential role of flattening filter free photon beams

Abstract: RapidArc treatment resulted in acceptable plan quality with superior ipsilateral tissue sparing compared to traditional techniques. Flattening filter free beams, recently made available for clinical use, might provide further healthy tissue sparing, particularly in contralateral organs, suggesting their applicability for large and complex targets.

“…The treatment of choice is radical surgery with wide safety margins. Because recurrences after surgery alone are observed in 40-80% of cases, RT plays an important role in achieving local control in the primary or postoperative setting [2,4,5,12,14,16,17,18]; indications for RT are primarily unresectable or recurrent tumors and non-in-sano resected tumors. RT has a long tradition in the treatment of desmoids: In 1928, James Ewing, who was the first to propose RT as a treatment option, described that desmoid lesions respond ‘slowly but satisfactorily' to RT [19].…”

“…For C-VMAT plans, 8 as shown in Figure 2A, double ipsilateral partial arcs with a maximum individual length of 240° starting from the mid-stermum were adopted in this study. In clockwise direction (CW), collimator angles were ranged from 15° to 30°, and 6MV photon beams were used.…”

“…Nevertheless, the influence of target motion on dose homogeneity and conformity degree will be increased with the increase of beam on times (BOT) for static gantry IMRT. 8,9 Several authors have investigated the application of volumetric-modulated arc therapy (VMAT) for whole or partial breast treatment. 1014 It is clear that VMAT may improve dosimetry and reduce treatment time compared with multiple-field IMRT.…”
