The aim of the study was to determine the frequency of different injuries caused by penetrating chest traumas, and also the cause and type of trauma and its accompanying injuries. This is a cross-sectional descriptive study, carried out on all patients referred to the emergency room of Shahid Bahonar Hospital, Kerman, from March 2000 to September 2008, due to penetrating chest trauma. The required information including age, sex, cause of trauma, type and site of injury, and accompanying injury was obtained and used to fill out a questionnaire and then was analyzed. 828 patients were included in the study; most of them were in the age range of 20-29. Of the patients, 97.6 % were males. The most frequent cause of trauma was stabbing, and the most frequent injuries following the trauma were pneumothorax and hemothorax. Orthopedic trauma was the most frequent accompanying injury. The most commonly used diagnostic method was plain chest radiography. In 93 % of the patients, the chest tube was placed and thoracotomy was performed for 97 % of the patients. Shahid Bahonar Hospital is a referral Trauma Centre and treats large number of chest trauma patients. Most patients need only chest tube placement as a definitive treatment.

Amaç: Travmatik hemotoraks nedeniyle kliniğimizde yatırılarak tedavi edilen olgularda tedavi yaklaşımları ve sonuçları değerlendi-rildi. Gereç ve Yöntem:Çalışmaya 2002-2011 yılları arasında, travma sonrası başvuran olgulardan, hemotoraks nedeniyle kliniğimize yatırılıp tedavi uygulanan 282 hasta dahil edildi. Olgular, yaş, cinsiyet, travma nedeni, uygulanan tedavi metotları, torakotomi endikasyonları, toraks travmasına ek olarak yandaş travmaların yeri, tedavi sonuçları açısından geriye dönük olarak incelendi. Bulgular:Motorlu taşıt kazaları en önemli hemotoraks nedeniydi (168 olgu, %59.5). 58 olguda (%20.5) delici-kesici alet yaralanması, 32 olguda da (%11.3) ateşli silah yaralanması olgusuna rastlandı. 165 olguda (%58.5) tüp torakostomi ile etkin ve yeterli tedavi sağlandı. On beş yaş altı olgu sayısı 22 (%7.8) idi. Acil torakotomi 35 (%12.4) olguya uygulandı. Olguların ortalama hastanede kalış süresi 8.3 gün oldu. Dokuz olguda (%3.1) mortalite, 43 olguda (%15.2) morbidite gelişti.

Özet AmaçBu çalışmanın amacı künt travmalı stabil hastalarda bilgisayarlı toraks tomografisinin klinik ve adli önemini araştırmaktı. Gereç ve YöntemlerTravma sonrası 24 saat içinde akciğer grafisi ve bilgisayarlı toraks tomografisi çekilen künt toraks travmalı 50 hasta retrospektif olarak incelendi. Hastaların dağılımı, torasik lezyonlar, tedavi yöntemleri ve adli sorunları incelendi. Bulgular Çalışmamızda, en sık tespit edilen lezyon kot fraktürü idi. Bilgisayarlı göğüs tomografisi torasik lezyonların tespitinde düz akciğer grafisine göre üstün bulundu. Altmışsekiz (%33) okült lezyon saptandı. Bu lezyonların 46 (%18)'sı düz akciğer grafileri ile saptanamayan hayatı tehdit edici lezyonlardı. Yandaş yaralanma 33 (66%) hastada vardı. Pelvis ve ekstremite kırıkları en sık olanlardı.Göğüs bilgisayarlı tomografisi pnömotoraks, hemotoraks ve akciğer kontüzyo-nunun tespitinde akciğer grafisine üstün bulundu. Seksenbir hayatı tehdit edici lezyon saptandı ve bu lezyonların 50'si (%61: pnömotoraks 13, hemotoraks 24, akciğer kontüzyonu 9 ve pnömomediastinum 4) düz akciğer grafileri ile tespit edilememişti. Klinik tedavi uygulaması 15 (%30) hastada ve adli yaklaşım 14 (%28) hastada değişti. Sonuç Göğüs bilgisayarlı tomografilerinin torasik travmalarda tedavi ve adli konularda titiz bir inceleme imkanı sağladığını düşünmekteyiz. Abstract AimThe aim of this study was to investigate the medical and forensic importance of thorax computed tomography in stable patients with blunt chest trauma. Material and MethodsFifty patients with blunt chest injury were retrospectively evaluated with chest radiography and thorax computed tomography in the first 24 hours after trauma. Patient demographics, thoracic lesions, management options, and forensic assessment were rewieved. ResultsThe most common lesion of the study was rib fracture. Thorax computed tomography was significantly superior to chest radiography in detecting thoracic lesions. Sixty eight (33%) occult lesions were detected. Forty six (18%) of these were life threatening that not detected with plain chest radiography. Associated injuries were present in 33 (66%) patients. Pelvic and extremity fractures were the most common injury. Thorax computed tomography was significantly superior to chest radiography in detecting pneumothorax , hemothorax and lung contusion. Eightyone life threatening lesions were detected and 50 (61%; pneumothorax 13, hemothorax 24, lung contusion 9, and pneumomediastinum 4) of these lesions could not be detected with plain chest radiography. The clinical management [in 15 patients (30%)], and the forensic assesment was changed [in 14 (28%)] patients were changed. ConclusionWe concluded that using Computed Tomography of the thorax in thoracic travmas prive meticulous assesment in management of patients and forensic issues.

Chest trauma in children

Abstract: The prevalence of chest trauma in children due to blunt injuries is high in Turkey. Extremity injury is thought to be the most commonly associated extra-thoracic injury. However, thoracic trauma in children can be managed conservatively in most of the cases.
