Chemotherapy‐induced peripheral neurotoxicity: A critical analysis

Abstract: With a 3-fold increase in the number of cancer survivors noted since the 1970s, there are now over 28 million cancer survivors worldwide. Accordingly, there is a heightened awareness of long-term toxicities and the impact on quality of life following treatment in cancer survivors. This review will address the increasing importance and challenge of chemotherapy-induced neurotoxicity, with a focus on neuropathy associated with the treatment of breast cancer, colorectal cancer, testicular cancer, and hematologica… Show more

“…[8] A higher incidence of paclitaxel neuropathy has been reported when paclitaxel is infused over three hours as compared to 24 hours. [11] We observed decreased incidence of neuropathy in the limbs treated with ice gloves and boots compared to controls during and post treatment with paclitaxel.…”

“…Most patients will have improvement of symptoms over months to years post treatment with a small number developing chronic, treatment resistant neuropathy. [8,9] There are no published reports on cold therapy as preventive strategy for chemotherapy induced peripheral neuropathy. Anecdotal reports by patients of improvement or disappearance of peripheral neuropathy symptoms with the use of cold therapy used to prevent nail changes with docetaxel, generated the hypothesis for this study.…”

“…Although cisplatin is one of the most remarkable anticancer drugs, its use is limited by severe side effects in normal tissues including neurotoxicity and nephrotoxicity, with an incidence of approximately 33% (Ries and Klastersky, 1986;Park et al, 2013). Several renoprotective approaches have been introduced such as inhibition of cisplatin uptake (Ciarimboli et al, 2005), inhibition of oxidative stress (Gao et al, 2013), and modulation of cell death pathways (Park et al, 2002).…”

“…The therapeutic effects are dose-dependent; however, high dose cisplatin is limited by cumulative nephrotoxicity (Pabla and Dong, 2008) and neurotoxicity (Park et al, 2013). Cisplatin primarily targets renal proximal tubular cells, inducing renal tubular cell damage and promoting nephrotoxicity through several mechanisms including oxidative stress, DNA damage, and inflammation (Clark et al, 2010;Pabla and Dong, 2008).…”

“…51 The incidence of thalidomide-induced PNP (TiPNP) is dependent on dose, treatment duration, and patient specific factors, and presents as a sensory neuropathy with prominent symptoms of paresthesia in the hands and feet (glove-and-stocking distribution). 49,52,53 TiPNP also includes numbness, and mild motor involvement with muscle cramps and weakness. 53 (Table 3).…”
