Chemotherapy for Fighting Schistosomiasis: Past, Present and Future

Abstract: Chemotherapy based on repeated doses of praziquantel remains the most effective control strategy against schistosomiasis, a neglected tropical disease caused by platyhelminths of the genus Schistosoma spp. Its long-term use, however, raises serious concerns about drug resistance against praziquantel. Therefore, it is generally acknowledged that alternative treatment options are urgently needed. This Review summarizes data on relinquished drugs as well as recent advances in the area of antischistosomal compound… Show more

“…However, there are concerns about the development of resistance to these drugs; triclabendazole resistance of P. westermani was reported in a human case from Korea [ 54 ]. Furthermore, there is widespread resistance to triclabendazole in liver flukes in cattle in Australia and South America [ 55 ], and praziquantel resistance is anticipated in the future owing to its widespread use as a single treatment for schistosomiasis, a worrisome situation that has encouraged the search for novel drugs [ 56 ]. The comparative analysis presented here identifies valuable putative protein targets for drug development, including Paragonimus -specific proteins and trematode-conserved proteins that do not share orthology to human proteins.…”

“…After penetrating the final host, cercariae develop into schistosomula (juvenile stage) and later into adult worms. [2,5] With respect to their relevance for human health, three speciesa re most common: S. mansoni, S. japonicum and S. haematobium. All three species localisei nd ifferentp arts of the body but cause similar symptoms.…”

“…[6] Praziquantel (PZQ) is the only drug that is widely used against all schistosome species (Figure 1). [2] An advantage of this drug is its efficacy toward all schistosome species and its low cost for treatment. [7] If 60 mg kg À1 of PZQ is administered orally on three consecutive days, a 90 %c ure rate can be achieved.…”
