Chemotherapy as Adjuvant Treatment for Intermediate-High Risk Early-Stage Endometrial Cancer

Abstract: Platinum-based chemotherapy is feasible and safe and it could be used in adjuvant setting for early-stage ECs. Although its effectiveness is comparable to radiotherapy, chemotherapy represents an excellent treatment option due to its systemic action. Further randomized studies will be needed to confirm our promising data.

“…The incidence of locoregional, distant, and locoregional/distant recurrence was 3.1%, 6.3%, and 2.1%, respectively, which was comparable to that reported previously. 7,8,20 Further, a similar DFS and OS were observed between the patients receiving 3 cycles of chemotherapy in our study and those with 6 cycles of chemotherapy in the report of Aloisi et al 20 Toxicity was observed in patients administered with adjuvant chemotherapy. Chemotherapy was delayed for 1 week in 3 patients because of grade 3 neutropenia.…”

“…Chemotherapy related toxicity was present in 18.2%, and radiotherapy-related toxicity in 1.9%. 13 These data are in line with the chemotherapy reported morbidity by Aloisa et al 18 Also, the percentage of late grade 3 gastrointestinal toxic effects related to EBRT are in line with data of the Portec-2 study. 17 The strength of this study is the fact that 2 treatment strategies were compared in 2 large well organized gynecological oncology centers.…”

“…Yet, none of these patients received both adjuvant treatments. 18 Although the importance of adjuvant radiotherapy for local control has been demonstrated in previous randomized, prospective trials, most studies excluded patients with FIGO stage IB and highrisk histology. 17,20 In a recently published study it was demonstrated that in these high-risk early stage EC patients (n = 51), adjuvant pelvic radiation was associated with improved vaginal control, pelvic control, and overall survival.…”

“…The management of endometrial cancer is primarily operative. It has emerged over the years from radical hysterectomy to more conservative surgery or conservative hormone treatment for early-stage endometrial cancer (2)(3)(4)(5), with hysterectomy and surgical lymphadenectomy only performed in deep myometrial invasion cases (6)(7)(8).…”
