Úvod: Základem léčby u časných stadií karcinomu laryngu jsou chirurgické přístupy zachovávající funkčnost hrtanu. U pokročilých stadií představuje totální laryngektomie významnou mutilaci pacienta a metodou volby je zde (chemo)radioterapie jako orgán záchovný postup léčby. Cílem sdělení je prezentace vlivu komorbidit a faktorů životního stylu na výsledky léčby v souboru pacientů ozařovaných pro karcinom laryngu v Ústavu radiační onkologie Fakultní nemocnice Bulovka (ÚRO FNB) v letech 2009-2018. Soubor pacientů a metody: V uvedeném období byla v ÚRO FNB provedena kurativní radioterapie pro karcinom laryngu u 189 pacientů. Konkomitantní chemoterapie (cisplatina 1× týdně) byla přidána u 48 (25,4 %) pacientů. Výsledky: Medián doby sledování činil 38,1 měsíců, medián doby do progrese 17,8 měsíce. Perzistence nebo recidiva onemocnění byly potvrzeny u 59 (31,2 %) pacientů. Byl prokázán velmi významný vliv komorbidit a faktorů životního stylu na léčebné výsledky. Nejzásadnější vliv má vstupní PS (performance status) a vstupní hodnoty hemoglobinu: pacienti vstupně v dobrém stavu nebo bez anémie vykazovali signifikantně delší přežití ve srovnání s pacienty ve zhoršeném stavu nebo s anémií. Prognostický význam má hodnocení komorbidit dle Indexu komorbidit dle Charlsonové a dle systému ACE-27. Abúzus většího množství alkoholu měl signifikantní vliv na horší výsledky. Závěr: V našem souboru pacientů s karcinomem laryngu bylo pomocí (chemo)radioterapie dosaženo dobrého léčebného efektu. Statisticky signifikantní vliv na celkové přežití byl zjištěn u vstupního PS, hladiny hemoglobinu, přidružených komorbidit a anamnézy abúzu alkoholu.
