Characteristics, Treatment and Outcome of Ischemic Stroke with Atrial Fibrillation in a Chinese Hospital-Based Stroke Study

Abstract: Background: There is scant information on characteristics, treatment, functional outcome and case fatality of ischemic stroke with atrial fibrillation (AF) in China. Methods: For this study, first-ever ischemic stroke patients who were admitted within 1 month of stroke onset during the period of March 2002 through December 2008 were included. Data on ischemic stroke patients were collected which included: demographics, risk factors, treatment administered, stroke-related complications and 3-month, 6-month and … Show more

“…18 Variation across hospitals in the Netherlands (29%-78%) was similar to our study. At a patient level, reports of functional dependence at 3 months poststroke hospital discharge in our study were similar to cohort studies from China, 19,20 Spain, 21 and Japan. 22 Importantly, these findings of dependence post-stroke in the United States and abroad demonstrate the high level of stroke morbidity and the need for continued efforts targeting functional recovery.…”

“…13,17,18 The previous population-based investigation for prevalence of RHD in China supported our results, which found 6 of 15 cases were underdiagnosed. 8 One possible explanation is that improvement in health care resulted in these subjects having their rheumatic fever treated earlier, which could have resulted in silent and/or less severe symptoms during the chronic phase 8 and, moreover, these patients were reluctant to receive echocardiography because of poverty.…”

“…12 Atrial fibrillation (AF) was defined as a history of persistent AF or paroxysmal AF, supported by past ECG or diagnosed by the attending physicians based on ECG and/or 24-hour ECG monitoring during admission. 13 Other vascular risk factors surveyed in this study included hypertension, diabetes mellitus, hyperlipidemia, coronary heart disease, previous transient ischemic attacks, history of stroke, and current smoking and alcohol consumption, which have been described in previous reports. 10,11 Stroke severity on admission was measured using the National Institutes of Health Stroke scale (NIHSS).…”

“…Older patients, heavy alcohol drinkers, and patients with higher National Institutes of Health Stroke Scale score on admission were independently associated with less frequent use of warfarin, whereas patients with history of heart failure and an INR between 2.0 and 3.0 during hospitalization were significantly associated with warfarin use at discharge. When compared with the previous studies from China that launched before 2010 8,21 (in which anticoagulants use in hospitalized AF patients were 11% and 14.2%, respectively), the rate of warfarin use was not increased substantially. In contrast, anticoagulation use among eligible AF patients improved from 88.4% to 95.2% during 7 years in the Get With The Guidelines-Stroke in the United States.…”
