Background Breast milk is the optimal choice for feeding premature babies. However, the prevalence rate of extrauterine growth restriction in preterm infants remains high. Objectives The purpose of this study was to analyze the macronutrients present in human milk and the correlation with the growth of in-hospital preterm infants. Methods This prospective study is based on data from 99 in-hospital preterm infants younger than 37 weeks of gestational age on an exclusively human milk diet. Infants who had previously received parenteral nutrition were eligible, but they had to have reached full enteral feeding at the time that the samples were taken. A total of 3282 samples of raw human milk or donor pasteurized milk were collected. The levels of lactose, protein, fat, and energy in the samples were measured using a Miris human milk analyzer. The primary outcome was weight growth velocity (g/kg/day) which was obtained using two-point approach. Results The mean (±standard deviation) macronutrient composition per 100 mL of milk was 7.2 (±0.3) g of lactose, 1.1 (±0.2) g of true protein, 3.5 (±0.9) g of fat, and 66.9 (±6.5) kcal of energy. The protein concentration in human milk had a positive, significant correlation with body weight gain, with a coefficient of 0.41 (p < 0.001). After adjusting for gestational age, postmenstrual age, small-for-gestational age, intraventricular hemorrhage, patent ductus arteriosus or congestive heart failure, duration of total parenteral nutrition support, bottle feeding or use of orogastric tube, and ventilator support, total daily protein intake was associated with body weight growth (p < 0.001).

Breastmilk is a natural, easy-to-digest food with high-bioavailability that meets the neonate's optimum growth and development needs in terms of the components it contains. Breastmilk and breastfeeding have many benefits for both the baby and the mother. However, some infants cannot be breastfed after birth for various reasons (mother's death, etc.). For such situations, the WHO, UNICEF and the Ministry of Health (Turkey) propose that another mother can breastfeed the infant. The concept of "mother's milk bank", which has been practiced in our country for many years, corresponds to this suggestion. However, the prevalence of diseases such as AIDS, hepatitis and the rapid growth of the formula industry have led to the abandonment of this traditional practice over the years. In many countries, mother's milk banks have been established to solve this problem. However, the few studies done in our country show that there are divergent views on the establishment of mother's milk banks. As a result, the possibility of creating an acceptable version of mother's milk bank in our country needs to be discussed. As an another result, instead of the mother's milk banks, "wet nurse centers" can be established to offer the same service in Turkey. Thus, the project of reviving "wet nurse and milk sibling" can be an effective solution. The aim of this study is to draw the attention of the community and the health personnel to the concept of wet-nurse and breast milk bank.

Yenidoğan sağlığı ve beslenmesi için kendi annesinin sütünden sonra en iyi seçenek bağışçı annenin sütüdür. Bağışçı anne sütünün temin edilmesi için, birçok ülkede yaygın olarak kullanılan yöntem ise, Anne Sütü Bankaları'dır. Bu nedenle, "Anne Sütü Bankacılığı" (ASB) çocuk sağlığı politikası ve düzenlemesi ile birleştirilerek, ulusal emzirme politikalarının bir uzantısı olarak korunmakta, geliştirilmekte ve desteklenmektedir. Yararları hakkında artan kanıtlara rağmen Türkiye'de gündeme gelen anne sütü bankacılığı uygulaması; etik sorunlar, geleneksel inançlar ve tutumlar, ailelerin bağışçı sütün güvenliği hakkındaki kaygıları ve bilgi yetersizliği nedeniyle tartışmalıdır. Bu derleme; ASB'nın amaç ve işleyişini, anne sütünün yenidoğan üzerindeki etkilerini, farklı kültürlerin anne sütü bankalarına yaklaşımlarını ve bağışçı sütü ile ilgili kültürel bakış açılarını içermektedir. Mother Milk Banks are the method commonly used in many countries for donors to provide breast milk. Mother milk Banking, unifying with the child health policy and regulaton in many countries. Despite growing evidence about the benefits of breast milk, In Turkey, implementation of milk banking is still under debate because of the ethical concerns, traditional beliefs and attitudes, parent's concerns about the safety of donor milk, and the lack of information. This review covers donor milk-related cultural perspective, impacts on the newborn and ASB's aims and functioning in the world, different cultures approach to the breast milk banks .

Abstract: Proper management and operation of a human milk bank can support breastfeeding, and provide a safe alternative to artificial formula for feeding preterm or ill infants in Taiwan. Sustainability of the milk bank needs more propagation and financial support by health authorities.
