Characteristics of Substance-Abusing Veterans Attempting Suicide: A National Study

Abstract: Demographic, diagnostic, and service utilization characteristics of veterans diagnosed with suicide attempt, substance dependence, both, or neither at discharge from Department of Veterans Affairs (DVA) hospitals in fiscal year 1994 (FY94) were compared using the DVA's discharge abstract database. Four groups of veterans were studied: (1) substance-abusing suicidal inpatients (n = 1,459), (2) substance-abusing nonsuicidal inpatients (n = 123,808), (3) nonsubstance-abusing suicidal inpatients (n = 632), and (4)… Show more

“…In clinical implications, treatment providers need to be aware of the high prevalence of suicide attempts in patients seeking SUD treatment (Anderson et al, 1995;Johnsson and Fridell, 1997), as well as the likelihood that a sizable minority of patients will attempt suicide in the year following treatment. Given the frequency of suicide attempt, careful screening and treatment planning in high-risk patients is particularly important for reducing the risk for subsequent suicidal behavior.…”

“…Consistent with the epidemiological evidence, previous suicidal behavior is common in patients who present for SUD treatment. In patients entering SUD treatment, anywhere from 8 to 45% report a lifetime attempt (Anderson et al, 1995;Johnsson and Fridell, 1997) and approximately 3 to 4% (Ilgen et al, 2005;Moos et al, 1998) report an attempt within 30 days of treatment entry.…”

“…It has been hypothesized that suicidal patients in treatment for their alcohol dependence may not benefit from the alcohol treatment as much as non-suicidal patients because of the excess psychiatric and substancerelated problems (15). A Finish study has shown that the treatment course of patients does not change after reporting of suicidal behaviour (37).…”

“…On the other hand, it is unclear how to plan the alcohol treatment so that the patients will obtain the desired benefit of the treatment. It has been hypothesized that suicidal patients have a poor outcome of alcohol treatment because of their comorbidity diagnosis (15), but the evidence supporting this is sparse.…”

“…According to some estimates, more than 40% of all patients seeking treatment for alcohol dependence report a suicide attempt at some point in their lifetime (Anderson et al, 1995;Koller et al, 2002;Roy, 2003). Additionally, impulsive suicide attempts are common in those with an alcohol use disorder (Ikeda et al, 2001) and may be especially important in understanding suicidal behaviors and the process of relapse in individuals with alcohol dependence (Conner, 2004).…”
