Lymph nodes with Hodgkin disease (HD)harbor few neoplastic cells in a marked leukocytic infiltrate. Since chemokines are likely to be involved in the recruitment of these leukocytes, the expression of potentially relevant chemokines and chemokine receptors were studied in lymph nodes from 24 patients with HD and in 5 control lymph nodes. The expression of regulated on activation, normal T cell expressed and secreted (RANTES), monocyte chemotactic protein (MCP)-1, macrophage inflammatory protein (MIP)-1␣, and MIP-1␤ was analyzed by in situ hybridization and that of CCR3 and CCR5 by immunohistochemistry and flow cytometry. It was found that, overall, the expression of all 4 chemokines was markedly enhanced, but the cellular source was different. RANTES was expressed almost exclusively by T cells whereas the expression of MCP-1, MIP-1␣, and MIP-1␤ was confined largely to macrophages. In control lymph nodes, chemokine expression was low, with the exception of MIP-1␣ in macrophages. CCR3 and CCR5 were highly expressed in T cells of HD involved but not of control lymph nodes. CCR3 was equally distributed in CD4 ؉ and CD8 ؉ cells, but CCR5 was associated largely with CD4 ؉ cells. In HD lymph nodes, CCR3 and CCR5 were also expressed in B cells, which normally do not express these receptors. All these chemokines and receptors studied, by contrast, were absent in the neoplastic cells. It was concluded that chemokines are involved in the formation of the HD nonneoplastic leukocytic infiltrate. Expression of CCR3 and CCR5 appears to be characteristic of HD, but the roles of these receptors' up-regulation for the disease process remain unclear. IntroductionHodgkin disease (HD) is characterized by the presence of few neoplastic mononuclear Hodgkin cells and polynuclear ReedSternberg (H-RS) cells surrounded by a dense nonneoplastic leukocytic infiltrate consisting of lymphocytes, plasma cells, granulocytes, and macrophages. 1 In the nodular sclerosis (NS) and mixed cellularity (MC) subtypes, CD4 ϩ T cells are particularly frequent in this infiltrate. Summary Twenty four patients with angio-immunoblastic lymphadenopathy (AILD) presenting between 1974 and 1985 have been reviewed. Clinical features at presentation included rash, fever, lymphadenopathy and hepatosplenomegaly in 75% of patients. Polyclonal hypergammaglobulinaemia was seen in 19/20 patients; 5 had normal immunoglobulin levels. Combination chemotherapy with MVPP was the optimal treatment with 6/7 patients achieving complete remission. Duration of remission ranged from 9 months to 4 years and was significantly longer in patients achieving complete as opposed to partial remission.In 6 patients phenotype studies were performed on single cell suspensions and immunoperoxidase studies on frozen sections of 7 lymph nodes. There was a reversal of the normal T suppressor/helper cell ratio with a predominance of T suppressor cells. Loss of normal B follicles was observed histologically in all except one lymph node. Germline configuration of the ,BB-chain of the T cell receptor was observed in lymph nodes of 4 patients with AILD, and a rearranged T cell receptor was observed in 1 patient in whom a second lymph node biopsy had shown alteration of the histological picture to that of T-zone lymphoma.Frozen sera of 15 patients were screened for antibodies to HTLV I and III and were found to be negative.

SUMMARYAdhesion molecules play an important role in the functioning of the immune system, particularly wilh regard to cell-tell interactions and antigen presentation. Several adhesion molecules are expressed on Hodgkin's disease-derived cell lines and these are important in their molecular interactions as antigen presenting celts (APC). There are no data regarding the expression of many of these adhesion molecules on Reed-Sternberg cells and ils mononuclear variant (Hodgkin's cells (HC)) present in pathologieal material. To obtain this infomiation we underiook an immunohistological study on material from 18 cases of Hodgkin's disease using a panel of MoAbs to examine the expression of adhesion molecules on HC. The HC were shown to express the integrin fi\ subfamily molecules. LFA-I (CO! la) and pi 50,95 (CDl lc) in high density but lacked CR3 (CDl I b). All of the immunoglobulin gene superfamily adhesion molecules studied were presenl to some degree on HC. wilh ICAM-2, in particular, showing moderate lo strong expression in most cases. The Hermes antigen CD44 was present in high density but leukosialin (CD43). another molecule present on diverse leucocyte types, was, in general, not detected on HC. These new data showing that ICAM-1, ICAM-2 and LFA-3 are, like LFA-1, expressed on HC emphasize the ability of HC to act as APC. The known adhesion molecule phenotype ofthe recently delined haematopoietic lineage ofhuman dendritic cells (DC) is broadly similar to that of HC. perhaps supporting the hypothesis that some HC represent a malignancy of an APC (DC) lineage.
