Characteristics of Group AStreptococcusStrains Circulating during Scarlet Fever Epidemic, Beijing, China, 2011

Abstract: Scarlet fever is one of a variety of diseases caused by group A Streptococcus (GAS). During 2011, a scarlet fever epidemic characterized by peak monthly incidence rates 2.9–6.7 times higher than those in 2006–2010 occurred in Beijing, China. During the epidemic, hospital-based enhanced surveillance for scarlet fever and pharyngitis was conducted to determine characteristics of circulating GAS strains. The surveillance identified 3,359 clinical cases of scarlet fever or pharyngitis. GAS was isolated from 647 of… Show more

“…A large ongoing outbreak of scarlet fever in Hong Kong and mainland China is being monitored (109)(110)(111). Starting in 2011, there was an alarming increase in the number of scarlet fever cases, including Ͼ1,500 cases in Hong Kong (110) and over 6,000 cases in Beijing (111).…”

“…Although GAS can be treated with ␤-lactam drugs, such as penicillin and amoxicillin, because of its universal susceptibility to the class, macrolides are alternatives for patients allergic to penicillins (1). GAS resistance to macrolides varies significantly by geographic location, and it has been increasing in some areas (16)(17)(18)(19)(20). In the United States, based on a multicenter study conducted about a decade ago, erythromycin resistance of GAS ranged from 3.8% to 4.3% in different years (16).…”

“…It is reported to be 8.2% in Germany (17) and 32.8% in Spain (18). It is highest in China, where more than 96% of GAS isolates are resistant (19,20). It has also been found that this is often clonally related, with certain emm genotypes predominant.…”

“…The emergence of a resistance emm 12 clone in China has aggravated the situation where 96% of GAS isolates were resistant to erythromycin (Yang et al, 2011). Resistance to tetracycline and fluoroquinolones has also been registered with 38% of the strains being resistant to the former globally (Ayer et al, 2007).…”
