Characteristics of dialysis-related amyloidosis in patients on haemodialysis therapy for more than 30 years

Abstract: Haemodialysis-associated amyloidosis was common in extremely long-term survivors. Even though the mean age at the start of haemodialysis was younger in the '30+' group than in the '20-30' group, the frequency of operations for CTS did not differ. This may be attributable to the recent advances in haemodialysis technologies.

“…Patients with end-stage chronic kidney disease (CKD) and maintenance haemodialysis treatment often present with an osteoarticular disease called renal osteodystrophy, which includes signs of secondary hyperparathyroidism, osteoporosis, osteosclerosis and osteomalacia [1][2][3][4][5][6]. Besides this, patients may complain of chronic pain and stiffness, with both large and small joints being affected symmetrically.…”

“…7,8) Some clinical studies reported that the development of clinical DRA was decreased with the use of this advanced HD technique. 7,9) The use of highflux non-cellulose membrane with increased permeability enables the removal of moderately sized molecules (10,000-15,000 Da), including many of the inflammatory proteins, β-2 microglobulin, and lipoproteins, from the body. 10,11) In this case, due to the long-standing HD history, the patient had been on traditional HD until the full-blown destructive DRA developed.…”

“…Furthermore, the prevalence of CTS correlates with the duration of hemodialysis [5,8,14]. Although these patients are likely to have less favorable symptoms and functional status after carpal tunnel release, it is unclear if patients who have release for hemodialysis-associated CTS have differing long-term relief of symptoms compared with patients with idiopathic CTS.…”

“…The prevalence of CTS correlates with the duration of hemodialysis [5,8], and CTS reportedly affects 32% to 50% of patients more than 10 years after the start of hemodialysis [8,14] and 80% over 30 years [14]. The reasons why patients receiving long-term hemodialysis have CTS develop are unclear.…”
