Ocular involvement is present in up to 79% of sarcoid patients. Uveitis is the main ocular manifestation and presents as a chronic intraocular inflammatory condition with potentially detrimental effects on visual acuity and quality of life. This retrospective study was conducted to explore the incidence and characteristics of ocular sarcoidosis in a single tertiary ophthalmology center. Medical records of 84 patients presenting between June 2007 and March 2021 were analyzed. Based on the “International Workshop on Ocular Sarcoidosis” (IWOS) criteria, ocular sarcoidosis was determined as: definite (n = 24; 28.6%), presumed (n = 33; 39.3%), probable (n = 10; 11.9%), and indefinite (n = 17; 20.2%) in our study population. In 43.9% of the definite and presumed cases, the eye was primarily affected. In addition to specific ocular findings, the diagnosis was supported by biopsy (28.6%) and chest x-ray or computer tomography (66.7%). Moreover, an increased soluble interleukin-2 receptor (sIL-2R) expression (76.2%), elevated angiotensin-converting enzyme (ACE) levels (34.8%), and lymphocytopenia (35.1%) were valuable laboratory findings. Co-affected organs were lungs (60.7%), skin (15.5%), and central nervous system (8.3%). Our findings support the prominent role of the eye in the early detection of sarcoidosis. In addition to the IWOS criteria, sIL-2R, in particular, was shown to be relevant in establishing the diagnosis.

Neurosarcoidosis is a rare complication of sarcoidosis and unusually presents as optic neuritis. We present the case of a 51-year-old man who complained of right vision loss. Brain magnetic resonance imaging showed asymmetrical enlargement of the right optic nerve. Chest computed tomography detected mediastinal and hilar lymphadenopathy. There were cutaneous nodules on the back. Biopsy of the mediastinal lymph node by endobronchial ultrasound-guided transbronchial needle aspiration and the skin showed noncaseating granulomas consistent with sarcoidosis. Serum angiotensin-converting enzyme level was elevated (34.2 IU/L) (normal: 8.3-21.4 IU/L). Based on these findings, he was diagnosed as neurosarcoidosis with optic neuritis. He was started on 1000-mg/day methylprednisolone intravenously for 3 days, followed by oral 50-mg/day prednisolone, which was gradually tapered for 8 weeks. Thereafter, the skin nodules and lymphadenopathy decreased and the right vision partially improved. Based on this rare case, sarcoidosis should be considered as a differential diagnosis in cases of optic neuritis.

Background: Ethnicity and geography have been described as determinants of the clinical presentation of uveitis and its etiology. Objective: To characterize patients from southwestern Colombia with uveitis. Material and methods: Retrospective study, from 2011 characteristics and outcomes of patients with uveitis were evaluated. Results: 144 patients were included. The age was 51.5 (35.2-61.7) years, 80 (55.5%) were women. 66% (n = 95) presented anterior uveitis. Of the clinical manifestations, the red eye was the most frequent, followed by ocular pain. Most had symptoms < 12 weeks, visual acuity of 20/20-20/40 and intraocular pressure ranges between 10-22 mmHg. Of the etiologies, 20 (17.9%) were still under study and 7 (6.3%) were undefined/idiopathic. Trauma was seen in 12 (10.7%), that associated with HLA-B27 in 10 (8.9%) and toxoplasmosis in 8 (7.1%). Conclusion: It is the first characterization of uveitis in southwestern Colombia.
