Changing Site Distribution of Colorectal Cancer in Japan

Abstract: These findings indicated that a proximal shift in the subsite distribution of colorectal cancer has occurred in Japan. This rightward shift of colorectal cancer is due to the decreasing proportion of rectal cancer. Furthermore, the increasing proportion of older patients, especially females, may be another major determinant of the changing colon cancer subsite distribution.

“…In Japan this shift was seen due to a decrease in the incidence of rectal cancer. [15,16] The influence of age of the patients on the subsite of the tumor was studied and it was found that incidence of colonic tumors was more followed by rectal and rectosigmoid tumors under each category of age groups considered and this association was statistically significant (p value <0.05). Also, as the age advanced, percentage of colonic and rectosigmoid carcinomas increased, while no such trend was seen in rectal cancers.…”

“…17 This has been noted worldwide in diverse populations. [21][22][23][24][25][26][27][28][29] In our Jewish population, over the last 3 decades we had observed a pattern of an initial increase in rectal cancer incidence in the lowest-risk population of Asian-African origin, followed by a decreasing incidence both in them and the high-risk European Jews. There is now a merged plateau incidence level of all long-term immigrant groups and Israel-born.…”

“…In developed countries, the incidence of proximal colon cancer increases with a time trend [4][5][6] . Furthermore, proximal shift of colorectal cancer is observed in the aged population [7][8][9][10][11] .…”

“…Recently, the incidence of colorectal cancer has been increasing remarkably in Japan and China, and nearly 37 000 deaths per year occur in Japan [2,3] . In these countries, there is a proximal shift in the subsite distribution of colorectal cancer [4][5][6] , which is associated with increase in age [7][8][9][10][11] . To reduce the high incidence, screening of colorectal cancer in asymptomatic individuals has been advocated [12][13][14] .…”
