Changing Patterns in the Practice of Carotid Endarterectomy in a Large Metropolitan Area

Abstract: Changes in the practice of carotid endarterectomy were studied by review of all endarterectomies performed in the greater Cincinnati area during 1980 and from July 1983 through June 1984. The number of operations rose from 431 to 750 (74% increase). The perioperative stroke rate fell from 8.6% in 1980 to 5.1% in 1983-1984; operative mortality declined from 2.8% to 2.3%; and the combined stroke or death rate declined from 9.5% to 6.5%. Asymptomatic carotid artery disease was the indication for 50% of the endart… Show more

“…The service volume influences the complication rate, as seen in other procedures. 19,20 The low complication rate associated with cerebral angiography may not be observed at other institutions. Therefore, the most appropriate application of these results is in high-volume settings similar to ours.…”

“…The overall frequencies of adverse outcomes were also higher in the 5 patients with complete ipsilateral carotid occlusions (40% T he benefit of carotid endarterectomy in comparison to medical therapy alone is highly dependent on surgical risk. 15 Postoperative complication rates higher than the 4% to 6% reported in randomized trials would eliminate the potential benefit of the operation. 69 Although several surgical series report perioperative complication rates of approximately 3%, 1 1317 A retrospective survey of all carotid endarterectomies performed at Johns Hopkins Hospital over a 10-year period found an 8.2% total rate of morbidity and mortality.…”
