Background— In heart failure with mid-range ejection fraction (HFmrEF) and preserved ejection fraction (HFpEF), feasible surrogate end points are needed for phase II trials. The aim was to assess whether a reduction in N-terminal pro–B-type natriuretic peptide (NT-proBNP) is associated with improved mortality/morbidity in an unselected population of HFmrEF and HFpEF patients. Methods and Results— In the Swedish Heart Failure Registry, HFmrEF (EF=40%–49%) and HFpEF (EF≥50%) patients reporting at least 2 consecutive outpatient NT-proBNP assessments were prospectively studied. Associations between reduction in NT-proBNP and overall mortality, HF hospitalization, and their composite were assessed by multivariable Cox regressions, with NT-proBNP changes modeled as binary (decrease/increase) or quantitative predictor by restricted cubic splines. In 650 patients, at a median of 7 months between the 2 measurements of NT-proBNP and over a median follow-up of 1.65 years, 361 patients (55%) showed a reduction and 289 patients (45%) an increase in NT-proBNP. Change in NT-proBNP was associated with risk of outcomes. Fifty-seven patients (16%) who decreased their NT-proBNP versus 78 patients (27%) who increased it died from any cause (adjusted hazard ratio=0.53; 95% confidence interval=0.36–0.77), 61 (17%) versus 86 (30%) were hospitalized for HF (hazard ratio=0.41; 95% confidence interval=0.29–0.60), and 96 (27%) versus 125 (43%) reported the composite outcome (hazard ratio=0.46; 95% confidence interval=0.34–0.62). These findings were replicated in HFmrEF and HFpEF separately. Conclusions— In HFmrEF and HFpEF during routine care, decreases in NT-proBNP were associated with improved mortality and morbidity. Studies to determine whether NT-proBNP changes in response to therapy predict drug efficacy are needed.

Обсуждается необходимость использования натрийуретических пептидов у больных с синдромом такоцубо. Обсуждается вопрос об ограни-ченной диагностической ценности использования натрийуретических пептидов у больных со сниженной функций почек, а также при дру-гих заболеваниях, при которых может увеличиваться концентрация натрийуретических пептидов в отсутствие сердечной недостаточности.Ключевые слова: сердечная недостаточность, натрийуретические пептиды, биомаркеры. A clinical role of biomarkers (especially, natriuretic peptides) is discussed in the article. The most important evidences of clinical effectiveness of using biomarkers in patients with suspected heart failure and in patients with confirmed diagnose of heart failure are reviewed. The need to use natriuretic peptides in patients with takotsubo syndrome is discussed. Limited diagnostic value of using natriuretic peptides in renal dysfunction, as well as in other diseases in which the concentration of natriuretic peptides may increase in the absence of heart failure, is also discussed.Keywords: heart failure, natriuretic peptides, biomarkers. ВведениеСовременную кардиологию невозможно себе представить без использования биомаркеров, позво-ляющих количественно оценить выраженность по-вреждения миокарда или его напряжение. Такие биомаркеры необходимы в определенных клиниче-ских ситуациях для установления или исключения ди-агноза инфаркта миокарда или сердечной недостаточности (СН). Использование биомаркеров позволяет у многих больных уточнить тактику лечения, либо отказаться от необоснованного применения те-рапии. Кроме того, оценка уровня соответствующих биомаркеров необходима для более точной страти-фикации риска развития осложнений заболевания сердца. В последнем варианте европейских клиниче-ских рекомендаций по тактике ведения больных с СН допускается, что в некоторых лечебных учреждениях может быть недоступно определение концентрации мозгового натрийуретического пептида (МНУП) в ка-честве стандартной тактики обследования больных с предполагаемой СН [1]. A szíveredetű natriureticus peptidek (B típusú natriureticus peptid, pre-prohormon B típusú natriureticus peptid) fontos szerepet játszanak a cardiovascularis homeostasisban, többek között vasodilatatiós, natriureticus, diuretikus és antiproliferatív hatásaik által. A natriureticus peptidek szakszerű mérésével dyspnoe esetén eldönthető, hogy respirá-ciós vagy cardialis eredetű, lehetséges a szívelégtelenség korai diagnózisa, a szívbetegség súlyosságának elbírálása (prognózis) és a kezelés hatékonyságának monitorozása. Sok országban mérése nagyon elterjedt akár a szakvizsgálat első lépcsőjeként a szív érintettségének eldöntésére. Eddigi elméleti és klinikai vizsgálatok nemcsak megerősítették, hogy a natriureticus peptideknek helye van a mindennapi betegellátásban, hanem napjainkra nemzetközi ajánlások is rendelkezésre állnak ezen biomarkerek gyakorlati alkalmazásáról. Kulcsszavak: BNP, NT-proBNP, szívelégtelenség, nehézlégzés, diagnózis Measurement of natriuretic peptides in heart failure: the good laboratory and clinical practiceCardiac natriuretic peptides (BNP, NT-proBNP) play a pivotal role in cardiovascular homeostasis, mainly due to their roles in vasodilatation, natriuresis, diuresis and due to their antiproliferative properties. Proper measurement of the natriuretic peptide levels may help differentiate between respiratory and cardiac forms of dyspnea, diagnose early forms of heart failure, evaluate severity of heart failure (prognosis) and monitor the effi cacy of therapy. In many countries natriuretic peptide levels are being used as one of the earliest diagnostics tools to evaluate the involvement of the heart. Current theoretical and clinical data confi rm the importance of natriuretic peptides in routine healthcare. These roles are clearly described in international recommendations and guidelines. In the current review the authors discuss the problems of the measurement of natriuretic peptides in Hungary, including several aspects related to laboratory medicine, cardiology and health economy.
