Changes in the Synovial Fluid Cytokine Profile of the Knee Between an Acute Anterior Cruciate Ligament Injury and Surgical Reconstruction

Abstract: Background: Changes in the intra-articular inflammatory state during the immediate period after an acute anterior cruciate ligament (ACL) rupture are not well defined. Purpose: To evaluate changes in the concentration of select proinflammatory and anti-inflammatory synovial fluid cytokines during the interval between an ACL injury and surgical reconstruction. Study Design: Descriptive laboratory study. Methods: In patients with an acute ACL injury, a synovial fluid sample was obtained from the injured knee dur… Show more

“…These inflammatory mediators mainly enter the bone tunnel through synovial fluid [ 44 ]. After ACLR, higher concentrations of inflammatory factors are detected in the synovial fluid, including Interleukin-1β (IL-1β), IL-6, Tumor Necrosis Factor-α (TNF-α), nitric oxide (NO) [ 14 , 45 ], which can cause osteolysis [ 46 ]. Some of the literature substantiate that a high level of the above substances in synovial fluid may be involved in the occurrence of TE [ 47 , 48 ].…”

“…TE is a common phenomenon, but little is known about relevant mechanisms. Synovial fluid, graft, and certain cytokines are main biological factors [ 13 , 14 , 15 ]. Mechanical factors, such as micromotion between tunnel and graft, fixation, bone tunnel location, may also be critical [ 16 ].…”

“…Its primary role includes recruiting mononuclear cells to sites of inflammation and the resulting inflammatory cascade throughout the body. 19,25 In rheumatoid arthritis, RANTES has been identified as a critical mediator in joint inflammation, and early work with rat models has demonstrated amelioration of arthritis when RANTES is competitively inhibited. 2,33 Systemically, increased RANTES expression has been linked to fibrotic immunoproliferative disorders such as sarcoidosis 26 and liver fibrosis.…”

“…Globally, there are over 2 million ACL injuries each year, with annual incidence rates continuing to rise, particularly in the young (2)(3)(4)(5). Although females are 2 to 8 times at higher risk for ACL injuries than men (6)(7)(8), it is not known if there are sex differences during early healing following a rupture (2,(6)(7)(8)(9). Some studies indicate that females may be more susceptible to functional deficits after ACL reconstruction (ACLR) surgery than men (10,11), however, few studies have focused on the impact of sex on early healing.…”

“…Some studies indicate that females may be more susceptible to functional deficits after ACL reconstruction (ACLR) surgery than men ( 10 , 11 ), however, few studies have focused on the impact of sex on early healing. This is an important clinical question because there is much controversy about how long one should wait before ACLR surgery ( 9 , 12 ). Too early can increase the risk of post-operative arthrofibrosis, and too late can increase the risk of meniscus tear and cartilage injury ( 1 ).…”
