Changes in Retinal and Choroidal Vascular Blood Flow after Oral Sildenafil: An Optical Coherence Tomography Angiography Study

Abstract: Purpose To describe changes in the retina and choroidal flow by optical coherence tomography angiography (OCT-A) after a single dose of oral sildenafil. Method A case-control study. Patients in the study group received 50 mg of oral sildenafil. Patients in the control group received a sham pill. Retinal and choroidal images were obtained at baseline (before pill ingestion) and 1 hour after ingestion. Central macular and choroidal thickness, choroidal and outer retina flow, and the retinal and choroidal vascula… Show more

“… 19 Other reports, however, show that sildenafil does not significantly increase choroidal blood flow. Berrones et al 20 used optical coherence tomography-angiography (OCT-A) to monitor ocular vascular changes and reported a significant increase in central choroidal thickness (CCT) and no change in choroidal blood flow in healthy patients one-hour following administration of 50 mg oral sildenafil compared to controls. They also reported a significant increase in outer retina blood flow and no change in retinal capillary density after sildenafil administration.…”

“…[10] Altered vascular perfusion is a known feature of CSCR associated with phosphodiesterase inhibitors (tadalafil, sildenafil); and a possible factor in mefloquine-associated CSCR. [211]…”

“…In the normal eye, the choroidal circulation nurtures the outer retina, whereas the central retinal artery and its end branches feed the inner retina and mid-retina. Due to the connection between these two systems (3,4), the bipartite vascularization ensures minimum supply for retinal hemodynamics (5). Animal models have shown that the choroid supply of oxygen to the inner retina may be sufficient to maintain ganglion cell viability even when the retinal vessels have been completely Experimental data has suggested that preconditioning events (prior episodes of ischaemia) may induce "ischaemic tolerance" in the retina and result in reduced anatomical and functional damage (6,7).…”
