Changes in Home Care Use by Disabled Elderly Persons: 1982-1994

Abstract: Use of formal sources of care by community disabled elderly residents increased, likely because of changes in the Medicare home health care benefits in 1989. Increased use of home health care was associated with the concurrent use of informal care.

“…The increase between 1984 and 1994 in the use of paid services among disabled community residents, including those with informal caregivers (Liu, Manton, and Aragon 2000;Spillman and Pezzin 2000), supports the possibility that average costs for the disabled rose. The increase affected all payers.…”

“…The increase affected all payers. The percentage of disabled elderly persons with formal caregivers who reported Medicare as a payment source rose from 16 percent in 1982 to more than 25 percent in 1994 (Liu, Manton, and Aragon 2000), and Medicaid programs also greatly increased their spending on community long-term care during this period. Out-of-pocket payments for freestanding home health care agencies also rose dramatically (Braden et al 1998;Letsch et al 1992).…”

“…Half the spending increase reflected higher average visits per recipient, but more than a third reflected an increased proportion of beneficiaries using the service (Komisar and Feder 1998). The percent of disabled elderly persons with formal caregivers who reported Medicare as a payment source rose from 16 percent in 1982 to more than a quarter in 1994 (Liu, Manton, and Aragon 2000). Medicaid programs over the same period have also greatly increased spending on community long-term care as an alternative to nursing-home care.…”

“…This is shown regarding the dependence in ADLs or IADLs (Activities/Instrumental Activities of Daily Living) in Canada (Crowell et al 1996;Shapiro and Tate 1997), the USA (Liu et al 2000;Miller and Weissert 2000), and Sweden (Thorslund et al 1991). Finnish researchers report that difficulties in mobility outdoors are predictors of formal care use (SakariRantala et al 1995).…”
