Changes in Circadian Variations in Blood Pressure, Pain Pressure Threshold and the Elasticity of Tissue after a Whole-Body Photobiomodulation Treatment in Patients with Fibromyalgia: A Tripled-Blinded Randomized Clinical Trial

Abstract: This study analysed circadian variation changes in blood pressure (BP), the pain pressure threshold (PPT) and the elasticity of tissue in patients with fibromyalgia (FM) after a whole-body photobiomodulation (PBM) treatment. This was a tripled-blinded randomized clinical trial including forty participants with FM. Participants using validated self-measurement BP devices attained readings that were used to calculate the circadian variation. Additionally, a standard pressure algometer of 1cm2 was used to assess … Show more

“…The treatment of superficial tissue uses wavelengths in the range of 600 to 700 nm, while the treatment of deeper tissue uses wavelengths in the range of 780 to 950 nm [ 1 , 2 ]. Currently, whole-body PBM has shown a systemic response in addition to the local response, with improvements in quality of life, pain, sleep disorders, tiredness, muscle spasm, morning stiffness, psychological factors, elastic properties of tissue, circadian rhythms, tender points, and in fibromyalgia sufferers [ 3 , 4 , 5 , 6 , 7 ].…”

“…A study evaluating whole-body PBMT in a group of female athletes resulted in better quality sleep and therefore reduction in quantity of sleep, even after higher training loads [104]. Augmented autonomic profiles seen after whole-body PBMT [96,104] has implications on sleep quality and vice versa [125,139].…”

“…For this reason, a comparison of our results with other studies is difficult. It is well known that as a consequence of an altered central nervous system, patients with FM exhibit neurovegetative symptoms and both central and peripheral sensitisation, as well as irritable bowel syndrome and the affected gynaecological area, chronic fatigue syndrome, postural orthostatic tachycardia, variations in circadian blood pressure or an excessive response to stimuli such as auditory, cold or mental stress [ 2 , 10 , 11 , 20 , 36 ]. Furthermore, the continuation of pain and symptoms leads to depression and affects mental health [ 37 , 38 ].…”

“…Even though the origin of FM remains unknown, current evidence defines it as a central sensitisation syndrome which involves an alteration in the hypothalamic–pituitary axis (HPA). Hence, ascending and descending pain pathways, as well as neurotransmitters, can be altered, which could be the cause of increased pain responses and the altered circadian variations in blood pressure in patients suffering from FM [ 10 , 11 ].…”
