Klinika tělovýchovného lékařství a kardiovaskulární rehabilitace LF UP a FN Olomouc Obezita je chronické metabolické onemocnění charakterizované zvýšením zásob tělesného tuku. Zvyšuje kardiovaskulární morbiditu a mortalitu, rapidně zatěžuje společnost nejenom socioekonomicky, ale především ze zdravotního hlediska. Důležitou roli ve vzniku kardiovaskulárních onemocnění hraje především typ tzv. "centrální-androidní-abdominální-viscerální" obezity. Tuk, nyní již chápaný jako hormonálně aktivní tkáň, se zapojuje do celé řady metabolických pochodů, řízení příjmu potravy, ovlivnění lipového metabolismu, průběhu zánětlivých procesů, produkuje řadu cytokinů, účastní se kontroly tělesné teploty atd. V poslední době je věnována velká pozornost ektopicky uloženému tuku, kam patří také epikardiální tuk (ET). Je to komplexní orgán, který je tvořen adipocyty, stromálními buňkami, mikrofágy, sítí nervových vláken a kapilár. Práce shrnuje aktuální poznatky o fyziologickém a patofyziologickém působení ET, měření ET, dále se věnuje významu ET ve spojení s častými onemocněními, jako je ischemická choroba srdeční, srdeční arytmie, diabetes mellitus, chronická obstrukční plicní nemoc a onemocnění štítné žlázy.Klíčová slova: arytmie, diabetes mellitus, epikardiální tuk, chronická obstrukční plicní nemoc, ischemická choroba srdeční, metabolický syndrom, obezita.
Clinical aspects of epicardial fat depositionObesity is a chronic metabolic disease which is characterized by an increased body fat. Obesity increases cardiovascular morbidity and mortality and presents an important socioeconomic burden for the health system. So-called "central-android-abdominal-visceral" obesity plays an important role in the development of cardiovascular diseases. Adipose tissue is nowadays considered to be metabolically active tissue which is involved in many metabolic processes, managing food intake, lipid metabolism regulation, and also inflammatory diseases. Many cytokines, such as those regulating body temperature, are produced by the adipose tissue. In the last few years, many studies of ectopic adipose tissue including epicardial fat were published. This is a complex organ which consists of adipocytes, stromal cells, macrophages, and network of neural cells and blood capillaries. This thesis summarizes updated information regarding physiological and pathophysiological role of epicardial fat, epicardial fat measurement and a possible connection with common diseases, such as ischemic heart disease, cardiac arrhythmias, diabetes mellitus, chronic obstructive pulmonary disease and disease of thyroid gland.
