Chairside treatment of amelogenesis imperfecta, including establishment of a new vertical dimension with resin nanoceramic and intraoral scanning

Abstract: Amelogenesis imperfecta is a hereditary disease affecting the structural development of tooth substance. This clinical report describes a 1-visit chairside treatment of an 8-year-old patient with amelogenesis imperfecta, using computer-aided design and computer-aided manufacturing (CAD-CAM) technology. Intraoral scanning was performed using the Cerec Omnicam. Thirteen resin nanoceramic crowns (Lava Ultimate) were fabricated chairside by using a Cerec MCXL milling unit and seated adhesively. The patient's treat… Show more

“…There have been many reports 1) on accidental fractures of dental ceramics due to small mechanical strength fractures. Mechanically strong ceramics, such as zirconia, have been used to prevent sudden fractures, and dental hard resins (HRs) with a high inorganic filler content and high toughness have been widely applied [2][3][4] . The International Organization for Standardization (ISO) has standardized the test methods for the mechanical properties of dental materials.…”

“…If an intact tooth requiring root canal therapy, BC mode can be used to restore the original tooth morphology after treatment. For tooth that has been repaired by a comfortable and satisfactory temporary crown for a long time, BC mode can also be used to make formal restoration according to the morphology of the temporary crown [15,19,30].…”

“…However, the standard automatic adaptation process is difficult for individual clinical defects. Thus, clinicians have to adopt manual design tools, making the design time-consuming and possibly affecting the strength of restorations [14][15][16][17][18][19][20].…”

“…This finding is in line with retention rates F I G U R E 2 Kaplan-Meier survival curves (blue) with 95% confidence intervals (red), censored restorations (+), and 36-month (▲) survival probability of other clinical studies, which investigated the survival of indirect restorations in MIH teeth. [21][22][23][24][25][26] The given outcomes support that long-lasting restorations on hypomineralized teeth can be achieved with conventional direct composite restorations 17,18 and indirect ceramic restorations. 21,39,40 It is important to note, however, that these treatment options can only be carried out in cooperative individuals or under general anaesthesia and in combination with removal of most of the clinically hypomineralized hard tissue and placement of the restoration margins, preferably on sound enamel.…”

“…Later, in life, when full cooperation is warranted, local anaesthesia, optimal cavity preparation, and restoration of the tooth can be achieved by the use of conventional composite restorations [18][19][20] or indirect restorations. [21][22][23][24][25][26] Nevertheless, both treatment options can also be conducted under general anaesthesia in case that dental rehabilitation is indicated in young, but uncooperative children. Interestingly, few clinical data 17 have been published about non-invasive clinical procedures in comparison with conventional restorations so far.…”
