Cesarean Scar Ectopic Pregnancy: The Lurking Danger in Post Cesarean Failed Medical Abortion

Abstract: Cesarean scar pregnancy (CSP) is a potentially life threatening ectopic pregnancy where a missed diagnosis is commoner than an accurate diagnosis. Incidence of Ectopic pregnancy is 1 – 2 % and cesarean scar ectopic occurs in about (0.05%) 1 in 2000 of all pregnancies. With increasing cesarean section rates worldwide, CSP is bound to increase with its dreaded complications like uterine rupture and catastrophic hemorrhage. “…CSPs pose significant risk of uterine rupture or dehiscence (a breakdown of the abdominal wall muscles which exposes internal organs to the environment) 6. With an incidence of 1 in 2000 normal pregnancies, these risks are not rare, and are increasing in frequency along with the popularity of caesarean deliveries 7. In addition to posing risk to the well-being of pregnant patients, as many as 60% of cases culminate in hysterectomy, rendering the patient sterile 8.…”

“…Some are estimated to be 1 in 100 pregnancies [5]; others are estimated at 20.70 per 1000 [6]. However, scar ectopic pregnancy is still considered a significant medical condition due to its potential for serious complications [7]. The risk factors for scar ectopic pregnancy include a history of previous cesarean sections or other uterine surgeries, such as myomectomy or cornual resection [8].…”

“…[10,11] Therefore, early diagnosis and effective treatment are key for reducing these risks in women with CSP. [12] The common symptom for CSP patients is usually little vaginal bleeding, occasionally accompanied by mild abdominal pain but most women are asymptomatic. So, diagnosis is mainly based on imaging examinations, namely, ultrasonography (US) and magnetic resonance imaging (MRI).…”
