Cervicomediastinal Giant Cystic Hygroma: A Case Report

Abstract: Cystic hygroma is a rare congenital malformation of the lymphatic system. It is most frequently detected in the head and neck region. Ultrasonography is performed as the first step for radiological diagnosis of these lesions. Magnetic resonance imaging provides important information for diagnosis and to guide treatment. In this paper, a case is reported of cervicomediastinal large cystic hygroma in a male infant.

“…Sclerotherapy is a common approach to treat hemangiomas and vascular malformations. Shou-jiang Tang et al [12] performed sclerotherapy for thoracic mediastinal LVMs by endoscopy, Omer Karakas et al [13] treated cervical-mediastinal LVMs with bleomycin and the lesion almost regressed. Compared with thoracotomy, sclerotherapy has many advantages, such as being minimally invasive, less operation impact on the patient, and low risk.…”

“…The cystic image is a result of the low density of the fluid in the inner site, though it may sometimes be perceived as a solid lesion rather than cystic if the inner site of the lesion is filled with hemorrhage, a high amount fat or protein that increases the density (10). Magnetic resonance imaging is the most sensitive radiological technique for the evaluation of the inner structure and the dimensions of the lesion, as well as its relationship with the near tissues, vessels and organs (11). The preferred treatment is total surgical excision, although some people prefer a conservative approach in asymptomatic patients.…”
