Cervical Ultrasound (US) and US-guided Lymph Node Biopsy as a Routine Procedure for Staging of Lung Cancer

Abstract: Routine ultrasound evaluation of supraclavicular lymph nodes reveals suspicious lymph nodes in a high number of patients with lung cancer. High-resolution US is superior to CT in the detection of pathological lymph nodes. Ultrasound-guided biopsy proves malignancy and thereby a N3 or M1 stage. Thus, more invasive and expensive diagnostic procedures can be avoided.

“…Lymph nodes farther in the cranial direction are defined as distant metastases (M1 station). Ultrasound is superior to both palpation and CT in identifying enlarged supraclavicular lymph nodes (Fultz et al 2002;Prosch et al 2007Prosch et al , 2008van Overhagen et al 2004).…”

“…Malignant nodes also appear as round to oval inhomogeneous structures with irregular margins and vascularization. They typically exhibit extracapsular growth and infiltration into vessels and the surrounding tissues (Prosch et al 2007). Despite those sonomorphologic criteria, the degree of malignancy remains uncertain.…”

“…2 Fractures of the rib (a fracture gap or a cortical step and a hematoma) can be seen well on ultrasound (Fig 2). Fracture diagnosis by ultrasonography is not only much more sensitive than conventional X-rays, but also demonstrates accompanying soft tissue lesions and easily a pleural effusion or a pneumothorax.…”

“…Der US ist sowohl der Palpation als auch der CT im Nachweis vergrößerter supraklavikulärer Lymphknoten überlegen [28]. Die höchste Prävalenz an supraklavikulären Lymphknotenmetastasen findet sich bei Patienten mit einem mediastinalen N 3-Stadium (44 % -51 %) oder bei Patienten, bei denen sich bereits Fernmetastasen finden (32 % -40%) [28,30].
