Although not all penetrating injuries to the neck need operative intervention, most patients with an aerodigestive tract injury should undergo operative repair. After initial stabilization of airway, breathing and circulation, a simple diagnostic approach should identify most patients with injuries. Most patients require simple repairs of injuries to the cervical oesophagus or trachea. Basic principles of proper exposure, debridement and tension free closures should provide the best outcomes and lowest complication rates.

Nonoperative management of both blunt and penetrating injuries can be challenging. During the past three decades, there has been a major shift from operative to increasingly nonoperative management of traumatic injuries. Greater reliance on nonoperative, or “conservative” management of abdominal solid organ injuries is facilitated by the various sophisticated and highly accurate noninvasive imaging modalities at the trauma surgeon’s disposal. This review discusses selected topics in nonoperative management of both blunt and penetrating trauma. Potential complications and pitfalls of nonoperative management are discussed. Adjunctive interventional therapies used in treatment of nonoperative management-related complications are also discussed.Republished with permission from:Stawicki SPA. Physical Examination and Arteriography in Patients With Penetrating Zone II Neck Wounds

Findings on physical examination are good predictors of arterial injury in patients with penetrating neck wounds and can exclude injury in over 99% of patients. Arteriography is a sensitive test but has a very low yield (1.4%). These findings question whether the current practice of mandatory neck exploration or proximity arteriography is necessary for patients without signs or symptoms of injury who have penetrating wounds of the neck.
