Cervical dystonia: clinical and therapeutic features in 85 patients

Abstract: -We studied patients with cervical dystonia (CD) to determine clinical features and response to botulinum toxin A (BoNT/A). Patients were submitted to clinical, laboratory and neuroimaging evaluation. BoNT/A was injected locally in 81 patients using electromyographic guidance. Four patients who had had previous treatment were considered to be in remission. The average ages at onset of focal dystonia and segmental dystonia were greater than for generalized dystonia (p<0.0003). The severity of the abnormal head-… Show more

“…Therefore, an important concern is the spread of CD to other body parts. Previous studies that evaluated clinical and demographic characteristics of patients with CD were not able to demonstrate predictable variables for the risk of CD progression [4][5][6][7][8] .…”

“…The pattern of CD may be of four types (torticollis, laterocollis, retrocollis or anterocollis) which may happen isolated (one type) or in combination (two or three types). Camargo et al 11 described a series with 85 patients with CD, and the most common clinical presentations were, according to the pattern: a) one type: torticollis (21 patients); b) two types: torticollis + retrocollis (19 patients); and c) three types: torticollis + laterocollis + retrocollis (09 patients). In our series, our results were different when the pattern of CD involved two or three types: a) two types: torticollis + laterocollis (12 patients in HT+ group and 17 in HT-); and in b) three type: torticollis +laterocollis + anterocollis (9 patients in each group).…”

“…Most voluntary muscles can be affected, and in the case of the neck muscles the condition is referred to as cervical dystonia (CD). CD is the commonest form of dystonia, and the majority of cases are focal 2,3 . The etiology of most cases of CD is unknown, but genetic and environmental factors are known to be implicated 3,4 .…”

Injections of botulinum toxin A (BoNT/A), a neurotoxin that has been extensively used to treat a variety of dystonic and non-dystonic movement disorders since the 1980s, is the treatment of choice nowadays for this condition 4,5 .…”
