Cervical disc herniation and cervical spondylosis surgically treated by Cloward procedure: a 10-year-minimum follow-up study

Abstract: BackgroundCervical degenerative pathology produces pain and disability, and if conservative treatment fails, surgery is indicated. The aim of this study was to determined whether anterior decompression and interbody fusion according to Cloward is effective for treating segmental cervical degenerative pathology and whether the results are durable after a 10-year-minimum follow-up.Materials and methodsFifty-one patients affected by single-level cervical degenerative pathology between C4 and C7 were surgically tr… Show more

“…Boni and Denaro [7] discussed this issue in 1982 but considered degeneration adjacent to a fusion at the cervical spine as clinically irrelevant. This was later confirmed by Hilibrand et al [40], who highlighted the role of natural history of the degenerated disc adjacent to the fused vertebrae, and by other authors [27], who outlined the role of postoperative cervical sagittal alignment in determining the accelerated degeneration of the disc adjacent to a fusion.…”

“…5,9 However, ACDF inherently decreases motion between the 2 fused vertebral segments, which has led to the advent of nonfusion techniques such as cervical disc replacement (CDR). 7,15 In the past decade, multiple studies have examined long-term outcomes 2, 5, and 7 years after ACDF and after CDR.…”

“…To fit this model, we used totally observed and expected new cases over 5 years. We also used varying slope model to estimate province-specific coefficients for covariates of the final model by using a CARNormal distribution (25).…”
