Cervical Chordoma in Childhood Without Typical Vertebral Bony Destruction

Abstract: Giant notochordal rest and benign notochordal cell tumors (BNCTs) need to be recognized for differential diagnosis of this atypical chordoma. A long-term follow-up might be necessary for the diagnosis of this nontypical patient.

“…Surgical resection of cervical chordomas is challenging because of the specific anatomic constraints of the cervical spine, including vertebral arteries, nerve roots, and pedicles 34, 49, 50. Moreover, chordomas tend to infiltrate into neighboring soft tissues 9, 31, 51. Experience with en bloc excisions for cervical chordomas is limited 31, 33, 50, 52, 53, and we have to perform intralesional piecemeal excisions of tumors in the upper cervical spine, in spite of multiple anterior/posterior surgeries.…”

“…The presence of an accompanying soft tissue mass, spanning several vertebral levels, is characteristic of chordomas ( 15 ). Soft tissue mass without bone involvement, as observed in 1 of the cases reviewed, has been previously described ( 15 18 ). MRI is useful in demonstrating the soft tissue mass.…”

“…Hemangiomas exhibit a polka dot appearance on axial images or a corduroy vertebra on sagittal images on CT scans. A number of cases without bone involvement have been described previously ( 15 18 ). In such cases, differentiation between chordoma and nerve sheath tumor may be difficult.…”

“…Chordomas rarely occur in people below 40 years old; however, numerous cases of pediatric chordomas have been reported (e.g., [4, 5]) and generally were associated with cranial locations [6]. While cranially located chordomas affect both genders equally, sacrococcygeal tumors are more frequent in males, with the male : female ratio being approximately 2 : 1 ([7], and references therein).…”
