“…In our case, the patient accepted two years of systematic rehabilitation for his spastic right upper limb, including CMIT, bilateral arm training and botulinum toxin injections, but the effectiveness was still limited. Some studies have reported that intrathecal baclofen (ITB) therapy as a reversible way can be applied to the patients with hemiplegic paralysis; it can reduce the spasticity in the lower limb and improve the walking ability [22,23] and also impacts the spasticity in upper limb equally as in lower limb [24]. However, baclofen for intrathecal injection is not available in China; thus, ITB was not considered in this case.…”

“…The catheter was placed into the lumbar subarachnoid space through a percutaneous technique. The level of the catheter tip was T10-11in all patients but the C7-T1 level was preferred in patients suffering from tetraparesis according to the literature (16). The pump was placed in the subcutaneous pocket in the lower quadrant of the abdomen and connected to the catheter.…”

“…46 There is evidence that a higher catheter tip location can result in improved upper extremity spasticity scores. 47 For pain treatment, placement of the catheter tip near the level of the pain may be more important when using analgesic agents that are lipophilic and penetrate further and fasting into biological tissue than do hydrophilic agents.…”
