Background Cervical cytology is essential for the early detection of cervical cancer. However, in Colombia, only 50% of women with subsidized health insurance were screened in 2019, compared to 100% of women with contributory insurance. This disparity highlights significant barriers that must be addressed. This study aimed to identify the factors that contribute to or hinder adherence to cervical cytology screening among low-income women with subsidized health insurance in a public primary care network in Cali, Colombia, from 2014 to 2018. Methods In a qualitative case study, the experience of women and health care and administrative personnel was recovered. Forty-seven women participated in seven focus group discussions. Five other women using the program participated in in-depth interviews. Finally, we interviewed eight people from the healthcare area and the health services administration. The qualitative data collected underwent content analysis, guided by the theoretical framework of Social Determinants of Health. Within this framework, five interconnected dimensions that influence adherence were incorporated. Results Adherence is a multifactorial phenomenon, and in relation to attendance at cervical cytology, the analysis delved into the mechanisms that affect it in a low-income context. Barriers to adherence were identified across multiple dimensions, including social and economic factors, health conditions, and patient-related factors, among both adherent and non-adherent women. Among adherent women, barriers and facilitators related to the healthcare team and system, as well as patient-related factors, were identified. Conclusions The findings of this research can be useful in developing personalized interventions and strategies to improve adherence and screening outcomes in low-income settings. It is necessary to increase the resources of health insurance entities to establish effective communication channels with women who attend the cervical cancer prevention program.

Objective: To evaluate the effectiveness of an educational intervention to improve knowledge and skills in taking Pap smear conducted by health staff implementing the program on Early Detection of Cervical Cancer. Methods: A quasiexperimental, longitudinal and prospective study, held from October 25 2010 to April 11, 2011 at a hospital in San Luis Potosi, Mexico. To select groups applied a non-probabilistic convenience sampling that was composed of 28 people, 15 for the study group and 13 to the control group. The study group received training that was divided into three phases in which assessed the knowledge and skills taking the smear, the control group received conventional training offered by your workplace. Papanicolaou quality was measured before and after the intervention. The reference used international classification system Bethesda 2001. For data analysis we used the Student t test. Results: The study group increased knowledge (t = 8.768, p = 0.000) in the management of official standards, in the anatomy of the lower female genital tract and in the form of testing. Technical skills (t = 8.639, p = 0.000) also increased in the study group after the intervention. The control group showed no significant changes. The quality report of the samples in the study group increased from 60% to 86.7% and in the control group decreased from 92.3% to 84.6%. Conclusions: The training is effective in improving the knowledge and skills in taking Pap. Health staff who received the training improved their performance and showed more interest in providing high quality service to women.

Objetivo: Determinar cuál es el nivel de conocimiento sobre la prevención del cáncer de cuello uterino en mujeres de 20 -35 años del Centro de Salud Madre de Calcuta en el Agustino Periodo Abril- Junio 2022. Métodos: Estudio Observacional descriptivo prospectivo de corte transversal. La población de estudio está conformada por ciento dieciocho mujeres entre las edad de 20-35 años del C.S de Salud Madre de Teresa de Calcuta, cuya muestra fue calculada por muestreo no probabilístico por conveniencia; El instrumento para la recolección de datos es una encueta que esta ya validada por un juicio de expertos dela universidad Wiener, utilizando el método según Herrera de 1998 válido porque alcanzo 0.79-0.99 de confiabilidad y validez. Resultados: El nivel de conocimiento sobre la prevención del cáncer de cuello uterino en mujeres de 20-35 años es deficiente con 50.85% y solo el 16.95% alcanzo un nivel de conocimiento bueno; el 67.80% de las encuetadas, alcanzo un nivel de conocimiento sobre la vacuna deficiente y el 10.17% logro un nivel de conocimiento sobre la vacuna bueno. El 42.37 % de las mujeres tiene un nivel de conocimiento con respecto al Papanicolaou y sus controles bueno y solo 26,27% tiene un nivel de conocimiento deficiente. El 68,49% de las mujeres, tienen un nivel de conocimiento con respecto a los métodos de barrera bueno, mientras que solo 30,51% tiene un nivel de conocimiento deficiente. Conclusiones: El nivel de conocimiento sobre las medidas de prevención sobre el cáncer de cuello uterino en mujeres de 20-35 años del C.S Madre Teresa de Calcuta solo alcanza un nivel de conocimiento bueno con 16.95% siendo este un resultado bajo, porque las encuestadas tienen algún tipo de conocimiento pero no han logrado captar en sí, todas las medidas de prevención que existen al respecto, con este resultado tan bajo no se puede cumplir las metas de la estrategia de prevención del cáncer de cuello uterino.
