Cerebrospinal Fluid Prostate Specific Antigen (CSF PSA) in Prostate Cancer Patients with Lower Spine Metastasis

Abstract: Although mean serum PSA and CSF PSA levels in the patients with cancer of the prostate and lower spine metastasis are higher than those in the others, the mean CSF PSA/serum PSA ratio is lower. However, clinical usefulness of CSF PSA value and CSF PSA/ Serum PSA ratio can be limited because CSF PSA values are usually very low, and CSF PSA/Serum PSA ratio of 4 prostate cancer patients are as high as 1 BPH patient.

“…The PSA concentration in the CSF is normally below detection limit, but may increase after metastastic spread of prostate carcinoma to the spine. However, only very moderate levels of up to 0.31 ng/ml have been reported [8]. Here we show that an intraspinal location of the metastastic tumor (without any radiological or cytological signs of leptomeningeal carcinomatosis) can increase the CSF PSA by 10 4 -fold more, with a CSF/ serum ratio of 1427.…”

“…Meningeal symptoms include headache, seizures, gait difficulties, sensory abnormalities and memory problems, as well as nausea and vomiting. Cranial nerve deficits are the most frequent signs, most commonly affecting the III, V, and VI PSA is not normally detectable in CSF, whereas in patients with prostate cancer, especially with skull or vertebral metastases, it can be found elevated, but its levels in serum are significantly higher [6]. In one study, the mean CSF PSA to serum PSA for patients without CNS involvement was 0.007 [7].…”

“…Given that PSA is present in cerebrospinal fluid (CSF), it can be hypothesized that PSA has a direct effect on brain tissue and brain function [27]. Significant differences in CSF PSA levels were observed between cancer and other prostate diseases [28]. However, in patients with PC, high CSF PSA levels are commonly associated with central nervous system cancer involvement/metastasis [29].…”
