Cerebral venous sinus thrombosis in traumatic brain injury: A systematic review of its complications, effect on mortality, diagnostic and therapeutic management, and follow-up

Abstract: ObjectiveCerebral venous sinus thrombosis (CVST) is increasingly being recognized in the setting of traumatic brain injury (TBI), but its effect on TBI patients and its management remains uncertain. Here, we systematically review the currently available evidence on the complications, effect on mortality and the diagnostic and therapeutic management and follow-up of CVST in the setting of TBI.MethodsKey clinical questions were posed and used to define the scope of the review within the following topics of compl… Show more

“…Dural venous sinus thrombosis (DVST) is an increasingly recognized condition in patients presenting with blunt traumatic brain injury and skull fractures [76]. When a skull fracture extends to a dural venous sinus or jugular bulb, it can lead to dural venous sinus thrombosis via direct mechanical disruption of the endothelium of the sinus wall [76].…”

When a skull fracture extends to a dural venous sinus or jugular bulb, it can lead to dural venous sinus thrombosis via direct mechanical disruption of the endothelium of the sinus wall [76]. Clinical presentations of DVST in the context of trauma can vary, and its clinical course may be insidious until subsequent complications arise, prompting medical attention.…”

“…Consequently, the role of hyperosmolar therapies (hypertonic saline and mannitol) as the first-line treatment to decrease intracranial pressure has been supported. If the situation continues to deteriorate, systemic anticoagulation, surgical decompression, or endovascular treatments, such as chemical thrombolysis or mechanical thrombectomy, may be considered [ 5 , 8 ]. Even though early diagnosis of this pathology has improved, the management of this condition still poses a significant challenge to the medical community.…”
