Cerebral Venous Sinus Thromboses in Patients with SARS-CoV-2 Infection: Three Cases and a Review of the Literature

Abstract: Introduction : Early studies suggest that acute cerebrovascular events may be common in patients with coronavirus disease 2019 (COVID-19) and may be associated with a high mortality rate. Most cerebrovascular events described have been ischemic strokes, but both intracerebral hemorrhage and rarely cerebral venous sinus thrombosis (CVST) have also been reported. The diagnosis of CVST can be elusive, with wide-ranging and nonspecific presenting symptoms that can include headache or altered sensorium… Show more

“…As previously discussed, the association of CVT with COVID-19 has attracted interest from neurologists. Table 1 summarizes the clinical characteristics of the CVT cases associated with CVT that have been reported [ [21] , [22] , [23] , [24] , [25] , [26] , [27] , 32 , [65] , [66] , [67] , [68] , [69] , [70] , [71] , [72] , [73] , [74] , [75] ]. “…1 Cerebrovascular complications occur in 0.5 to 5% of acute COVID-19 infections. [2][3][4][5] Acute ischemic stroke comprises the majority of cases, but intracerebral hemorrhage, 6 cerebral venous sinus thrombosis, 7,8 the posterior reversible encephalopathy syndrome (PRES) 9 and two cases of RCVS 14,15 have also been reported. There are several mechanisms by which COVID-19 infection may lead to stroke, including exacerbation of pre-existing underlying vascular risk factors, viral mediated hypercoagulability, hyperinflammatory state, general critical illness, hypoxia, hypotension, direct cardiac effects, but also via activation of the renin-angiotensin system, and direct endothelial invasion leading to endotheliitis.…”

“…While pulmonary embolism (PE) and deep venous thrombosis (DVT) have been well documented in patients with COVID-19, there is limited information on CVST associated with COVID-19 [ 4 ]. We present a case of CVST due to COVID-19 in a 30-year-old Indian man who presented to the ED with a headache one week after he was diagnosed with COVID-19 pneumonia.…”
