Cerebral astrocytomas: histopathologic correlation of MR and CT contrast enhancement with stereotactic biopsy.

Abstract: Gadolinium-labeled diethylenetriaminepentaacetic acid was used as a contrast agent for stereotactic magnetic resonance (MR) imaging in six selected patients with brain tumors who underwent stereotactic biopsy. Regions of contrast enhancement demonstrated by computed tomography (CT) and MR imaging in four of the six patients correlated with areas of malignant neovascularity and endothelial proliferation within solid tumor. Radiation necrosis produced contrast enhancement indistinguishable from that of recurrent… Show more

“…However, the relationships between histological findings and the signal intensity changes according to the scanning technique employed must be clarified to determine the extent of tumor resection. For example, areas of neovascularization and endothelial proliferation appear as regions enhanced by gadolinium-DTPA, 8) and tumor cell infiltration is located within the area of high signal intensity on T 2 -weighted fast SE imaging. 10) Such issues have not yet been fully investigated.…”

“…The enhancing portion of the lesions corresponds to regions where the blood brain barrier was disrupted and was often highly vascularized (8). Regions of contrast enhancement have also been reported to correlate with areas of malignant neovascularity and endothelial proliferation (9,10).…”

“…For gliomas, the extent of tumor is often larger than the Gadolinium enhancing region and isolated tumor cells have been found within intact white matter outside contrast enhancement (9). Although the high signal intensity on T2 weighted images is typically larger than the enhancing region, it cannot be considered representative of the lesion because it does not differentiate between tumor and edema.…”

“…Male to female ratio is 3:2 Glioblastoma Multiforme (GBM) are more common in elderly (10). All GBM are associated with oedema, necrosis intralesional haemorrhage and tumor margin are indistinct and they stand out using IV contrast (11,12,13) . Butterfly GBM can extend across corpus callosum (fig -4) Second common intra axial neoplasm was metastases (fig -2).…”
