Cerebral Abscess Complicating Proteus mirabilis Meningitis in a Newborn Infant

Abstract: Neonatal bacterial meningitis is associated with high morbidity and mortality, especially when complicated by intracranial abscesses. The authors report the case of an 11-day-old female with Proteus mirabilis meningitis complicated by an asymptomatic brain abscess. With this report, the authors illustrate the importance of surveillance neuroradiologic imaging in patients with Proteus meningitis.

“…There are several species of Proteus, but Proteus mirabilis and Proteus vulgaris are more common among clinical isolates. It is the causative organism of urinary tract infections, osteomyelitis, mastoiditis and wound infection as well as neonatal gram-negative meningitis with an incidence of 4% (8). As a complication, intracranial abscess has an increased risk of neurologic sequelae, developmental delay, visual and audiologic impairment or death (9).…”

“…Brain abscesses were found when the imaging was performed at least a week after disease onset. The finding of a brain abscess on brain MRI imaging on initial anti biotic use 4) , seizure occurs 7) , or when the antibiotics do not match the culture sensitivities 2) . For our patient, we chose cefotaxime and vancomycin as initial antibiotic empirical treatment for bacterial meningitis.…”

“…Route of infection has not been established but presumed to be ascending infection during delivery due to its early presentation. In most of the cases reported in literature, the course of the infection appears to be fatal involving severe neurological complication with cerebral abscess and pneumocephalus formation despite institution of appropriate antibiotics [2][3][4][5].…”
