Background and Aim: Cleft lip and cleft palate are among the most common orofacial abnormalities. Patients with these deformities commonly present with midface deficiency and need challenging treatment modalities that focus on improving the position of the maxilla. Tongue plate appliance is an intraoral device that has shown promising results in the treatment of growing patients with maxillary deficiency. Nonetheless, the effects of tongue plate on patients with cleft lip and cleft palate have not been evaluated yet. This study aimed to assess the effects of tongue plate on growing patients with cleft lip and palate. Materials and Methods: Twenty-four growing patients (12 girls and 12 boys) with non-syndromic unilateral cleft lip and cleft palate between the ages of 6-12 years volunteered to participate in this quasi-experiment. All the patients had undergone the preliminary stages of lip and palate closure during infancy, but none of them had received bone grafts. They were treated with tongue plate appliance for 18±3 months. Lateral cephalograms were obtained and were analyzed at the beginning and at the end of the treatment. Paired t-test was used for statistical analysis of normally-distributed data; otherwise, Wilcoxon test was used. Results: Paired t-test showed that the Sella-Nasion-Point A (SNA) angle was increased from 76.3±0.2 to 77.3±0.14 degrees, and the Point A-Nasion-Point B (ANB) angle was enhanced from-2.26±0.17 to 0.93±0.82 degrees (P<0.001). Conclusion: Tongue plate appliance has shown favorable results in the treatment of class 3 malocclusion and maxillary deficiency in growing patients with cleft lip and cleft palate.

The aim of this study is to present the importance of interdisciplinary treatment approaches for the patients with clefts. In this study, the treatment of four patients with cleft lip and palate were presented. After the completion of fixed orthodontic treatment, proper overjet and overbite relationships were achieved and to obtain the esthetic and functional results, two of the patients had prosthodontic treatment (one of them has removable appliance and another has fixed partial denture). Two of the patients had composite restoration on lateral incisors. Patients with cleft lip and palate require interdisciplinary treatment approaches. Functional occlusion and good dental esthetics can be achieved with a team approach, well-established diagnosis and correct treatment plan.

Cleft lip and palate (CLP) is the most common congenital deformity of the orofacial. Clefts are thought to be of multifactorial etiology due to genetic and environmental factors. Different dental abnormalities are usually seen in cleft patients, including midface deficiency, collapsed dental arches, malformation of teeth, hypodontia, and supernumerary teeth. Moreover, feeding and speech are major functional dilemmas for those patients. The goal of treatment is to restore esthetics and functional impairments associated with clefts. The nature and the extent of medical and dental problems among CLP patients dictate the need toward multidisciplinary approach where different medical and dental specialists are involved in the treatment. The purpose of this section is to codify and synthesize a literature about management of cleft lip and palate deformity from birth until adulthood so that general concepts, principles, and axioms can be formulated. In this regard, feeding plates, nasoalveolar molding (NAM), lip and palate repair, palatal expansion, alveolar bone grafting, rhinoplasty, orthodontic treatment, and orthognathic surgery will be discussed. Furthermore, the question of proper timing for each therapeutic procedure is scrutinized in this chapter. Suggested clinical tips and changes of treatment modalities are summarized and illustrated as well.
