Central venous thrombosis and thrombophilia in cystic fibrosis: A prospective study

Abstract: Our data do not support biological screening at the time of a TIVAD insertion. We emphasise the contribution of a medical history of venous thromboembolism and prospective Doppler-US for identifying asymptomatic catheter venous thrombosis to select patients who may benefit from biological screening and possible anticoagulant therapy.

“…16 Two studies reported their findings from prospective studies evaluating both children and adult patients with CF, all with central venous catheters and found the rate of VTE to be 3.7-6.6%. 11,17 Similarly, Raffini et al reported a high incidence of VTE (27%) in patients with a history of BCC.…”

“…The previous studies that have quantified the rates of VTE in patients with CF have varied in duration from 2 years to 13 years [14][15][16] with two of the studies being prospective observations. 11,17 The three retrospective reviews that are most similar to our patient population included adults with CF who had central venous catheters and reported VTE rates from 3.5% to 16.1%. [14][15][16] Nash et al also evaluated the rate of VTE in patients with a positive culture for BCC.…”

“…In patients with CF, a spontaneous pneumothorax should also be highly suspected in case of a sudden onset chest pain, especially if they have respiratory distress. Cardiovascular disorders such as pulmonary embolism may also be evocated, particularly in patients with central venous catheter (CVC) . Myocardial infarction is rare and has only been described anecdotally.…”

“…Evaluation for aminoglycoside ototoxicity in 63 patients found abnormalities when tested via standard pure tone audiometry (n = 13), extended high‐frequency audiometry (n = 15), and distortion‐product otoacoustic emissions (n = 12), demonstrating differences in prevalence depending on assessment method . In a single center prospective study, thrombophilia and hypercoagulability were found in half of the 90 patients who received a totally implantable vascular device, but catheter venous thrombosis frequency was low at 6.6% …”
