ABSTRACT:The aim of present study was the installation of permanent jugular catheter to miniature pigs, which are frequently used as animal model for the experiments closely related to human medicine. We describe here in many details surgical interventions leading to the localization and fixation of Seldinger needle in v. jugularis externa and its use for extended period of time. Eight animals were included in these experiments and their heath status was currently monitored and no visible problems were recorded. After two months they were euthanased and potential function of catheters was carefully inspected. Only in two instances we found pathological changes resulting in the obstruction of catheter and trombophlebitis of v. jugularis externa sin and v. cava cranialis. In six remaining animals, it was easy to inject any time the solutions with drugs to blood system or to take safely blood samples. During whole post operation period the animals were maintained in conventional conditions, without any special care.

Giriş: Böbrek yetmezliği olan hastalarda acil hemodiyaliz genellikle kateter takılarak sağlanmaktadır. Hemodiyaliz kateter uygulamalarının erken dönemdeki komplikasyonları morbidite ve mortaliteyi arttırabilir. Bu çalışmada kliniğimizde uyguladığımız geçici hemodiyaliz kateter olgularının özelliklerini ve komplikasyonlarını inceledik.Yöntemler: Çalışmamızda 2012 Ağustos-2013 Ağustos tarihleri arasında kliniğimiz nefrologları tarafından geçici hemodiyaliz kateteri takılmış olan 151 olgu incelendi. Hastaların demografik özellikleri, kateter takılma endikasyonları, kateter takılma yerleri değerlendirildi, komplikasyonlar kaydedildi. Bulgular: Doksan dördü kadın, 57'si erkek, toplam 151 hasta çalışmaya dahil edildi. Hastaların yaş ortalaması 58, 66±16,8 (minimum: 17, maksimum: 92) idi. Kateter takılması sırasında 112 hastada (%74,2) herhangi bir komplikasyon gözlemlenmedi. Otuz dokuz hastada (%25,8) çeşitli nedenlerle tekrar kateter uygulaması yapıldı. İlk defa kateter takılan 151 hastanın 12'sinde (%7,9) arter ponksiyonu gelişti. Cerrahi girişim gerektiren hasta olmadı.Sonuç: Acil hemodiyaliz gereksiniminden dolayı geçici kateter uygulamaları gereklidir. Ancak pek çok komplikasyon riskini beraberinde getirir. Hemodiyaliz gereken hastaların erken dönemde nefrolojiye yönlendirilmesi ve kalıcı vasküler girişimlerin uygulanması geçici kateter gereksinimini ve komplikasyonları azaltacaktır.Anahtar Sözcükler: Geçici kateter uygulamaları, komplikasyonlar, arter ponksiyonu Aim:Urgent hemodialysis in patients with renal insufficiency is usually provided by catheter insertions. Early complications of hemodialysis catheter insertions can increase morbidity and mortality. In this study, we investigated the characteristics of patients undergoing temporary hemodialysis catheter insertion in our nephrology clinic and complications. Methods:In this study, we reviewed the records of 151 patients in whom temporary hemodialysis catheters were inserted by the nephrologists of our clinic between August 2012 and August 2013. Demographic features, catheter insertion indications, and catheter insertion sites were assessed and complications were noted.Results:A total of 151 patients (94 female and 57 male) were included in the study. The average age of the patients was 58.66±16.8 years (minimum: 17, maximum: 92). No complication was observed in 112 (74.2%) patients during catheter insertions. Catheter insertion was repeated in 39 (25.8%) patients for various reasons. Arterial puncture occurred in 12 (7.9%) of 151 patients who had catheter insertions for the first time. None of the patients required surgical intervention. Conclusion:Temporary catheter insertions are required in patients needing urgent hemodialysis. However, they bring along the risk for many complications. Early referral of patients who require hemodialysis to nephrology clinic and application of permanent vascular interventions will reduce the need for temporary catheters and complications.

Профилактика катетер-ассоциированных инфекций в онкопедиатрии РЫКОВ МАКСИМ ЮРЬЕВИЧ, ГЬОКОВА ЕЛЕНА ВИТАЛЬЕВНА, СУСУЛЕВА НАТАЛЬЯ АЛЕКСАНДРОВНА, ПОЛЯКОВ ВЛАДИМИР ГЕОРГИЕВИЧ Выраженный успех в лечении онкологических заболеваний у детей, достигнутый в последнее десятилетие, объ-ясняется внедрением мультимодальных программ лечения, в которых ведущее место отводится химиотерапии. Ее эффективность связана не только с развитием фармакологии, но и с интенсификацией протоколов лечения -увеличением дозировок химиопрепаратов, количества курсов и сокращением интервалов между ними. Это, в свою очередь, требует сложной сопроводительной терапии для профилактики и коррекции осложнений и по-бочных эффектов. Поскольку основной способ введения как химиопрепаратов, так и сопроводительных -внутри-венный, это накладывает жесткие требованиям на системы центрального венозного доступа. Они должны быть надежными, простыми в использовании, устанавливаться однократно на весь период лечения, иметь минималь-ный процент осложнений при установке и эксплуатации, а также не снижать качества жизни пациентов. Такие условия особенно важны в онкопедиатрии, поскольку любая катетеризация центральных вен у детей -инвазив-ное вмешательство, требующее общей анестезии. В настоящее время всем перечисленным требованиям отвеча-ют имплантируемые венозные порт-системы.Ключевые слова: детская онкология, катетер-ассоциированные инфекции, имплантируемые венозные порт-системы, химиотерапия.
