Central venous catheter-related blood stream infections in patients receiving intravenous iloprost for pulmonary hypertension

Abstract: Catheter-related blood stream infection (CR-BSI) in patients with pulmonary hypertension (PH) receiving intravenous iloprost via an indwelling central line has previously not been fully described. Recent studies have suggested a link between the pH of prostanoid infusions and the rate and nature of CR-BSI. We have investigated CR-BSI in patients receiving intravenous iloprost at our unit. Databases and hospital records were interrogated for all patients receiving intravenous iloprost between September 2007 and… Show more

“…While studies have documented the CLABSI rates among PH patients receiving IV prostanoids, 8 12 , 14 , 15 few studies have analyzed pediatric PH populations. 17 This study examines not only the types of infections and infection rates in children receiving IV prostanoid drugs, but also the effect of line management strategies on reinfection.…”

“…2 The incidence of CVL-BSI among adults treated with PGI 2 analogues is 0.1-1.13 cases per 1,000 catheter-days. 5,12 The REVEAL registry report is the largest, with 1,063 patients >18 years old with at least 5 years of follow-up (the number of CVL-days was not given). 2 As noted above, the overall rate of CVL-BSI was 0.20 cases per 1,000 line-days (epoprostanol ¼ 0:12, treprostinil ¼ 0:36), with more gram-negative infections associated with the latter analogue.…”

“…prostanoid are at risk of developing a catheter-related bloodstream infection (CRBSI). 12 Patients with a CRBSI will often have no external signs of line infection and may present with non-specific worsening of breathlessness. An elevated, unexplained, CRP in such patients should lead to exclusion of a line infection with peripheral and central blood cultures and antibiotic therapy to cover gram-positive and gram-negative organisms while cultures are pending.…”
