Cellular bases for human atrial fibrillation

Abstract: Atrial fibrillation (AF) causes substantial morbidity and mortality. It may be triggered and sustained by either reentrant or nonreentrant electrical activity. Human atrial cellular refractory period is shortened in chronic AF, likely aiding reentry. The ionic and molecular mechanisms are not fully understood and may include increased inward rectifier K+ current and altered Ca2+ handling. Heart failure, a major cause of AF, may involve arrhythmogenic atrial electrical remodeling, but the pattern is unclear in … Show more

“…23 Rapid atrial activation during atrial fi brillation shortens the action potential and the refractory period of atrial cardiomyocytes, reduces the length of re-entry wave λ and causes electrical heterogeneity. 24 These changes turn into arrhythmia substrate and facilitate its persistence. This makes the arrhythmia itself a major provoking moment for its persistence.…”

“…3 Chronic ventricular tachypacing (VTP) models of HF-induced AF have been shown in numerous studies to cause atrial structural and electrophysiological remodelling, with a variety of changes in ion currents, action potentials (APs), and intracellular Ca 2+ (Ca 2+ i ) handling. For example, AP duration (APD) is either increased, 4,5 unchanged, 6 or shortened; 7 the Ca 2+ i -transient amplitude is increased 5 (in contrast with a decrease, in the ventricle), 8 and delayed afterdepolarizations (DADs) also occur.…”
