Cell-laden alginate hydrogels for the treatment of diabetes

Abstract: Introduction: Diabetes mellitus is an ever-increasing medical condition that currently suffers 1 of 11 adults who may have lifelong commitment with insulin injections. Cell-laden hydrogels releasing insulin may provide the ultimate means of correcting diabetes. Here, we provide insights of this cell-based approach including latest preclinical and clinical progress both from academia and industry. Area covered: The present article focuses on reviewing latest advances in cellladen hydrogels both from the technol… Show more

“…Several alginate analogs have been used to enhance biocompatibility. These analogs do not trigger strong inflammatory responses and possess similar behavioral characteristics under similar conditions [ 165 ]. For example, chitosan and alginate can tolerate acidic pH and do not show toxic effects during in vitro and in vivo studies [ 166 ].…”

“…Consequently, management of obesity requires a comprehensive approach, among which dietary modifications and physical activity play the most important role. Observational as well as interventional studies show that the increased consumption of dietary fiber is associated with weight loss, and alternative treatments with dietary fiber have made their way up to clinical trials, with various degrees of success [ 165 ]. Given its effects in reducing inflammation and postprandial blood glucose, alginate has been considered as an agent in the treatment of obesity.…”

“…As previously discussed, macroencapsulation devices have some limitations due to their complicated designs, low mass transport, and limited cell response ( Espona-Noguera et al, 2019 ). Furthermore, the slow diffusion of oxygen into this device may cause delays in insulin release leading to hypoglycemia ( Izeia et al, 2020 ). However, macrodevices are safe; they can reduce the risk of teratoma formation, cells’ survival, and their secreted insulin for months.…”

“…Due to this difficulty in the transfer of microcapsules to larger animals, and the risk of thrombosis in case of intravascular macrocapsules, extravascular macroencapsulation strategies, often based on alginate, are currently regarded as the most promising option as summarised in a recent review [ 23 ]. In general, macroencapsulation approaches have a number of advantages for the transplantation of pancreatic islets, such as the possibility for a high islet density and easy retrievability.…”
