Cefotaxime diffusion into cerebrospinal fluid of children with meningitis

Abstract: Cefotaxime diffused consistently and in therapeutic levels into the cerebrospinal fluid (CSF) of 13 children successfully treated for bacterial meningitis. CSF cefotaxime levels early (6.0 ,ug/mI) and late (1.2 ,ug/ml) in treatment were severalfold the MBCs for the infecting organisms. After a single 40-mg/kg dose to each of five infants with ventriculostomies, mean CSF levels of cefotaxime were 6.4, 5.7, and 4.5 ,ug/mI at 2, 4, and 6 h, respectively.

“…It has been demonstrated that cefotaxime has effective penetration into the cerebrospinal fluid (4,32). The concentration of cefotaxime in cerebrospinal fluid after administration of a dosage of 200 mg/kg/day, similar to the doses used in this study, ranges from 1.5 to 6.0 g/ml during the acute phase of meningitis (24).…”

“…When AUC,O,CSF was related to AUCOs, the AUC ,OcsF/AUC-o_Os ratio of cefotaxime was 0.03 to 0.21 (median = 0.12; n = 5) compared with 0.006 to 0.018 (median = 0.007; n = 5) for ceftriaxone. antibiotics achieved concentrations in CSF several times higher than those in the present study (1,3,4,6,9,10,16,20,22,30). Elimination of both antibiotics in CSF was considerably slower than that in serum.…”

“…As shown in Results, the median cefotaxime penetration into the CSF was 0.28, which was similar to the findings of a study in children with meningitis (8). However, a large interindividual variability of the penetration was demonstrated.…”
