Cecal Volvulus a Rare Cause of Intestinal Obstruction. A Case Report

Abstract: Cecal volvulus (CV) is a rare cause of acute intestinal obstruction caused by torsion or twisting of a mobile cecum and ascending colon. Early diagnosis and management are essential to prevent serious complications such as bowel gangrene, cecal perforation, and generalized peritonitis. We report a case of cecal volvulus with impending cecal perforation.

“…The pathogenesis of cecal volvulus is multifactorial and may involve congenital abnormalities, the presence of redundant colon loops, or post‐surgical adhesions 5 . The clinical signs and symptoms can vary greatly including common symptoms like severe abdominal pain, constipation, nausea, vomiting, and abdominal distension 6 . Prompt diagnosis is of utmost importance because delayed recognition and treatment can lead to severe consequences such as bowel ischemia, gangrene, and bowel perforation.…”

Although cecal volvulus is indeed a rare occurrence among adolescents, high clinical suspicion should be maintained, when a patient present with right lower abdominal pain and an unconventional postoperative recovery.…”

“…Cecal volvulus has variable presentation ranging from recurrent abdominal pain, acute abdominal pain, abdominal distension, absolute constipation, vomiting, and shock. Recurrent abdominal pain is seen in cases of mobile cecum syndrome, which can progress to acute cecal volvulus as well as can resolve spontaneously 6,11–14 . Acute cecal volvulus presents with acute abdominal pain with a clinical picture similar to acute intestinal obstruction rapidly progressing to strangulation and possible perforation leading to hemodynamic instability and shock 15,16 …”

“…Cecal volvulus is the twisting or rotation of a hyper-mobile cecum around its mesentery, which ultimately causes intestinal obstruction. If it remains undiagnosed, it may cause impairment of blood supply, intestinal perforation, and fecal peritonitis [ 6 ].…”
