Background: Early and effective identifi cation of childhood neurodevelopmental disorders remains a critical task of all pediatric healthcare professionals, which is critical to the well-being of children and their families. Methods: A retrospective review of medical records of all preschool children referred to a Child Development Centre (CDC) in NorthWest England, over a six-month period between Sept 2014 and Feb 2015 was conducted. The local multi-professional approach to the clinical assessment and management of preschool children was described and the published literature on this topic was reviewed. Results: Twenty four different categories of professionals spanning the whole range of primary, secondary, and tertiary healthcare, social care and educational services were involved in the management of the patients. The largest group of professionals was the primary healthcare specialists. The ten different primary care professionals managed an average of 42% of the patients. The secondary healthcare providers were involved in the care of an average of 17%, tertiary care providers 10%, educational specialists 25% and social care professionals were involved with 5% of all the patients. The commonest diagnostic disorders were Speech/Language delay (56%), Global developmental delay (33%), Behavior diffi culties (26%), Social communication concerns (21%) and Autistic spectrum disorder (19%). Conclusion: The high number and specialties of various healthcare professionals at all levels of care indicates the high social and economic investment required in managing the affected preschool children in the region. Childhood neurodevelopmental disorders in the preschool age represents a high level of public health signifi cance.

Introduction: Multiple neurodevelopmental problems affect 7-8% of children and require evaluation by more than one profession, posing a challenge to care systems. Description:The local problem comprised distressed parents, diagnostic processes averaging 36 months and 28 visits with 42% of children >4 years at referral to adequate services, and no routines for patient involvement. The co-design project was developed through a series of workshops using standard quality improvement methodology, where representatives of all services, as well as parents participated.The resulting integrated care model comprises a team of professionals who evaluate the child during an average of 5.4 appointments (N = 95), taking 4.8 weeks. Parents were satisfied with the holistic service model and 70% of children were under 4 at referral (p < 0.05). While 75% of children were referred, 25% required further follow-up by the team. Discussion:The Optimus model has elements of vertical, clinical and service integration. Reasons for success included leadership support, buy-in from the different organisations, careful process management, a team co-ordinator, and insistent user involvement. Conclusion:Evaluating multiple neurodevelopmental problems in children requires an integrated care approach. The Optimus care model is a relevant showcase for how people-initiated integrated care reforms can make it into usual care.

This study aimed to assess and identify developmental delays in infants using the Denver Developmental Screening Test (DDST-II), examining potential contributing factors and offering recommendations for effective interventions. MethodsThe study employed a prospective cross-sectional design, enrolling 111 infants aged 0 months to 2 years attending a tertiary care center for 2 years. Denver Developmental Screening Test (DDST-II) was administered, and infants were followed up for three consecutive months to assess developmental milestones and potential contributing factors. ResultsIn the study encompassing 111 infants, 6.4% demonstrated delayed development, 7.8% exhibited doubtful delays, and 85.8% were considered normal. Notably, 25% of cases with developmental concerns had a history of preterm birth, 35% were of low birth weight, and 30% were delivered via cesarean section. Furthermore, 100% detection rates for delayed development were observed in infants aged 13-24 months. ConclusionThe current study underscores a notable prevalence of developmental delays among infants, emphasizing the critical importance of early detection and intervention. The multifactorial nature of contributing elements, including preterm birth and low birth weight, highlights the complexity of developmental challenges. The study's findings contribute valuable insights into developmental patterns, supporting the need for broader research and targeted interventions. RecommendationThe study recommends further research with larger and more diverse samples to enhance generalizability. Additionally, exploring the effectiveness of tailored interventions for developmental delays is crucial for optimizing infant development.
