Cavernoma of the septum pellucidum in the region of foramen of Monro

Abstract: A rare case of cavernoma in the region of the septum pellucidum is reported. A 35-year-old female patient presented with chronic headaches. Her neurological exam was normal. Her magnetic resonance (MR) imaging showed a lesion within the inferior aspect of the septum pellucidum extending into the anterior third ventricular region, blocking the foramen of Monro, resulting in moderate supratentorial asymmetrical hydrocephalus. A central neurocytoma or subependymoma was suspected on imaging. Complete excision of t… Show more

“…10 studies were selected using the PRISMA flow diagram and were included in the final analysis. The literature included 10 patients with CMs in the septum pellucidum (Table-1) with a median age of 42 years (range: 16 to 74 years) [4, 3, 5, 6, 711]. Of these patients, 8 were male (80%) and 2 were female (20%).…”

“…The severity of such clinical features depends on the location and the extent of the size of the entity. Seizure is a common presentation in supratentorial cavernoma, approximately 39-79% of patients with an annual risk of development of 1.5% [7,17,18] In contrast, bleeding is more common in the infratentorial region with a prevalence of 3.8% compared to the supratentorial region, which bears a prevalence of 0.4%. Our analysis demonstrates hemorrhagic complications in 30% and seizure manifestation in 10% of cases, which is quite the opposite of the statistics of supratentorial cavernoma.…”

“…Despite all technological imaging improvements, the precise relationship between the lesion and the ventricular structures and the clot identification, in our case, was best observed by endoscopic view. The microsurgical approach is the more advocated treatment for IVC [ 3 , 10 , 11 ]. Modern microsurgical techniques guarantee a safe and complete resection of IVC, but a high rate of postoperative neurological deficits is still associated with these procedures [ 3 ].…”

“…6 A CV arising from the septum pellucidum is a rare entity, and only four cases have been reported in the international literature. 7 8 9 10 Generally speaking, CVs may be detected incidentally or present with seizures, headaches, or focal neurologic deficits due to hemorrhage. The clinical features largely depend on the location of the CV.…”
