Causes and age of neonatal death and associations with maternal and newborn care characteristics in Nepal: a verbal autopsy study

Abstract: Background In Nepal, neonatal mortality fell substantially between 2000 and 2018, decreasing 50% from 40 to 20 deaths per 1,000 live births. Nepal’s success has been attributed to a decreasing total fertility rate, improvements in female education, increases in coverage of skilled care at birth, and community-based child survival interventions. Methods A verbal autopsy study, led by the Integrated Rural Health Development Training Centre (IRHDTC), … Show more

“…Per the study findings, more than half of the early neonatal deaths occur during the first two days after birth. This finding was parallel with studies conducted in Ethiopia [ 60 ] and Nepal [ 61 ]. This implies that the measures taken in the first two days after birth could substantially reduce early neonatal death.…”

“…This reflects the most to least common causes of neonatal mortality in 2017 in Nepal as determined by the WHO and Maternal and Child Epidemiology Estimation Group (MCEE) [19]. This is in contrast to the study results of Erchick et al, which examined causes of neonatal deaths across six districts in Nepal from 2012-2013 [20]. Among the 338 neonatal deaths, sepsis was the leading cause of death (47%), followed by birth asphyxia (16.6%) and prematurity (13.3%).…”

“…Physicians bring a more holistic approach to the assignment of cause of death, but have interobserver reliability issues that are removed by the use of an algorithm. Algorithms are reproducible by definition but are not necessarily more valid [20]. Of infants born preterm <37 weeks gestation, 2/3 were assigned prematurity as a COD.…”

“…More efforts are therefore necessary for low- and middle-income countries to achieve the United Nations Sustainable Development Goal 3 target 3.2 which aims to end all preventable deaths of newborns and children aged under 5 years of age by 2030 ( 3 5 ). Nepal remained with the 20 th highest rate of PTB in the world in 2010 ( 6 ), and prematurity has often been reported as one of the major causes of neonatal deaths in Nepal ( 7 9 ).…”
