Catheterization Without Supination—A Series of 36 Prone Position Internal Jugular Vein Cannulations

Abstract: OBJECTIVES: Describe the clinical characteristics and outcomes of 32 critically ill patients who underwent central venous cannulation of the internal jugular vein while in prone position. DESIGN: Retrospective cohort analysis. SETTING: Single tertiary-care urban academic safety-net hospital. PATIENTS/SUBJECTS: Patients requiring mechanical ventilation and prone positioning for severe acute respiratory distress syndrome from March 1, 2020, through March 31, 2021. “…In 2022, our working group published a series of seven cases of VA in PD, three of which were placed in the internal jugular vein ( 6 ). We consider that it would be important to analyze some points on which we agree with Lussier et al ( 1 ). First, the placement of VA in PD seems to be feasible and safe, since, as has been described, the number of complications such as pneumothorax, bleeding, and arterial punctures are almost nil, especially when performed under ultrasound guidance.…”

“…81 It is worth noting that central venous catheterization can be successfully and safely undertaken during PP when performed by well-trained staff with carefully selected patients. 92 Additionally, in situations where immediate supination is challenging or poses unacceptable risks to the patients, cardiopulmonary resuscitation can also be performed in the PP. 93…”

“…Right internal jugular vein is the preferred site due to its ease of and reduced risk of complications, 6,7 followed by left internal jugular vein, femoral vein and subclavian vein. Use of real-time Ultrasonography increases the success rate of venous puncture and reduces risk of arterial puncture.…”
