Catecholamine-resistant hypotension and myocardial performance following patent ductus arteriosus ligation

Abstract: Objective We performed a multicenter study of preterm infants, who were about to undergo patent ductus arteriosus ligation, to determine if echocardiographic indices of impaired myocardial performance were associated with subsequent development of catecholamine-resistant hypotension following ligation. Study Design A standardized treatment approach for hypotension was followed at each center. Infants were considered to have catecholamine-resistant hypotension if their dopamine infusion was >15 µg/kg/min. Ech… Show more

“…The other documented cause of circulatory compromise following PDA ligation is poor vascular tone, likely associated with the presence of adrenal insufficiency. This group of patients does not respond to dopamine or epinephrine and, in most of these patients, hydrocortisone reverses the circulatory compromise [53]. Unfortunately, pre-ligation assessment of adrenal function does not identify this group of patients.…”

“…Cardiac dysfunction characterized by poor myocardial performance and decreased contractility is one of the underlying causes in a number of patients [51,52]. In this group, the most widely used vasopressor-inotrope, dopamine, appears to be effective in improving the circulatory compromise [50,53]. Prophylactic use of milrinone in a subset of patients with CO < 200 mL/kg/min has been reported to reduce post-ligation hypotension compared to historical controls [54].…”

“…However, there are some recommendations in the literature regarding the use of milrinone and dobutamine in the newborn, as well as some reservations, which led us to question their efficacy in the newborn period. 1,2,5,6,8,9,[12][13][14][15][16] In the current study, the aim was to understand the efficacy of milrinone and dobutamine when used on preterm infants with PLCS clinical signs developing after PDA ligation and which of these two medicines had a greater therapeutic effect. While doing this, the parameters of patient follow-up methods most commonly used in neonatal intensive care units (NICU) were considered.…”
