Catching a resurgence: Increase in SARS-CoV-2 viral RNA identified in wastewater 48 hours before COVID-19 clinical tests and 96 hours before hospitalizations

Abstract: Curtailing the Spring 2020 COVID-19 surge required sweeping and stringent interventions by governments across the world. Wastewater-based COVID-19 epidemiology programs have been initiated in many countries to provide public health agencies with a complementary disease tracking metric and facile surveillance tool. However, their efficacy in prospectively capturing resurgence following a period of low prevalence is unclear. In this study, the SARS-CoV-2 viral signal was measured in primary clarified sludge harv… Show more

“…6 ). The amount of intra-laboratory precision attained for PMMoV provides confidence and has been suggested to be useful to normalize SARS-CoV-2 RNA concentration estimates for the detection of temporal trends (e.g., D’Aoust et al., 2021 ). However, the use of a consistent method and laboratory for each system/community is again underscored and recommended to prevent avoidable methodological biases.…”

“…WWTP influent is assumed to be a well-mixed sample of contributions from the human population and previous studies in our laboratory [32,33] have demonstrated that human fecal markers are at steady-state levels in WWTP influent, but for these markers, nearly every human in the service area is contributing. In the case of SARS-CoV-2 infections, only a small number per 100,000 people are contributing SARS-CoV-2 based on clinically diagnosed cases, albeit these data likely underestimate prevalence [34]. We note that our analysis of consecutive days examined primarily large sewersheds servicing more than 50,000 people.…”

“…The resulting best fit curve had an R 2 value of 0.987, with slope and intercept of −3.203, and 38.987 respectively. Calculated primer efficiency under these conditions was 105.2%, which meets the requirement to be an efficient primer (usually 90-130%)[41].…”
